 What do I need to do before I start?

You will receive a welcome letter via the US Postal Service or when you arrive for your pre-employment physical.

Your offer letter package will contain a selection of new hire forms and policies that you should review and complete prior to your start date.

 What should I bring to my pre-employment appointments?

Pre-employment physical appointment in Associate Health & Wellness:

 Photo identification

 Immunization records

Paperwork session in Human Resources:

 Identification for the Federal Form I-9 (form and instructions are included with your offer letter)

 Direct deposit information such as a voided check or deposit slip

 Original copies of any educational documents or licensure required for the position

 Completed new hire paperwork

 What happens during my pre-employment physical?

The pre-employment physical will consist of a physical according to the requirements of the position, a drug screen, a TB skin test and verification of MMR titers.

The Associate Health and Wellness Manager will go over your medical history and the job description

 Please note that your TB skin test will need to be read within 48-72 hours after being placed. If it is more convenient to have the test read at another location, a form can be provided at the time of your physical. If you have documentation of a TB skin test having been performed within 90 days of the date of your physical, please bring that with you to your appointment. If you have questions before the physical, please contact the Associate

 Where can I learn more about benefits?

If your position is at least 0.5 FTE (20 hours per week) or greater, you are eligible to enroll in benefit plans which include medical/dental/vision coverage, life insurance, retirement savings, and much more. If you are eligible, and are interested in these benefits, you have 31 days from your date of hire in order to enroll. Please note: If you would like to take advantage of our benefits package, and you do not do so within the 31-day time period, you will not be eligible to enroll until the next annual enrollment period.

Representative will be happy to assist you.

You can also wait to complete your paperwork until you arrive in HR for your appointment. Once there, you will have the opportunity to meet with an HR professional who can answer your questions and help you through the paperwork process.

 I was offered a sign-on bonus. How will I be paid? What are my responsibilities?

Bonus agreements are personal, and details vary according to each individual situation. For that reason, specific details about your sign-on bonus will be given in the recruitment bonus agreement. This bonus agreement will either be provided by Human Resources or be included directly in your offer letter. Bonuses are either paid in a lump sum or in installments over time, depending on your specific agreement, and are subject to the specific terms outlined in your recruitment bonus agreement. If you did not receive or have misplaced your bonus agreement, contact Human Resources for a duplicate copy. It is important that you keep a copy of this agreement for your records.

 I’m moving here from out-of-state to take this position. Will I be reimbursed for relocation expenses?

In certain very specific situations, Wheaton Franciscan Healthcare offers reimbursement for relocation costs. Not all positions or situations are eligible, and it is important to note that reimbursement considerations are at the discretion of the hiring leader and Human Resources. For specific guidelines and eligibility requirements, please contact your Human Resources Representative.

 I’m also a student. Can Wheaton Franciscan Healthcare help me with my tuition costs?

Wheaton Franciscan Healthcare offers a tuition reimbursement program for current benefit-eligible associates. To qualify, you must be an associate at 0.5 FTE or greater, have been employed with Wheaton Franciscan

Healthcare for at least 6 months, and be free of written warnings for the 12-month period prior to the first day of class. The course must be taken for credit at an accredited institution, and must relate to your current position or possible future positions within Wheaton Franciscan Healthcare. Accreditation is validated by HR Connect through the U.S. Department of Education at http://ope.ed.gov/accreditation .

You must also submit paperwork and documentation for reimbursement within 3 months of the course end date. Depending on your FTE level, you may be eligible to receive reimbursement of up to $2,000.00 per fiscal year for fees, tuition, and costs directly related to the course.

Professional certificate programs offered for credit and non-academic credit are also eligible for reimbursement.

Professional certificates and related coursework will be covered at 100 percent to a maximum of $500 per fiscal year The last day of class or the day in which the coursework for the certificate program is completed determines the fiscal year in which the maximum is applied
