DIVISION: Medical Centers DEPARTMENT:________________ BASIC NATURE OF POSITION: Responsible for coordination and management of day-to-day operation of the program, ensuring compliance with requirements of the grant. Responsible for education and training to support the integration of primary, mental and behavioral health care and the use of evidence-based practice. Responsible for providing mental health services for the prevention, diagnosis and treatment of mental, behavioral and emotion disorders individuals, families, couples and groups. Responsible for the coordination and tracking of patients referred for complex/specialty behavior health care to ensure continuity of care. 2. Ex: Ex: Non-Ex: Non-Ex: EDUCATION EXPERIENCE: A. Formal education required: Masters degree with LMSW certification from an accredited program; ACSW preferred. B. Experience/skills/ability required: Minimum of three (3) years direct clinical experience, including two (2) years in case work methodology and experience with children, adults, individual and family counseling in a health care setting. Ability to perform psychosocial assessment risk factor analysis and treatment planning. C. Further formal education which is not required but which may be desirable: None. Specialized training and/or certification/license: Certified by the Academy of Certified Social Workers (ACSW) 4. PRIMARY RESPONSIBILITIES: A. Coordinate and manage day-to-day operation of the program, ensuring compliance with requirements of the grant. Educate and train to support integration of primary and behavioral health care and use of evidence based practice. B. Coordinate tracking patients referred for complex/specialty behavioral health care to ensure continuity of care. C. Provide mental health services for the prevention, diagnosis and treatment of mental, behavioral and emotion disorders in individuals, families, couples and groups. D. Knowledge of biological, psychological and social development theories, diversity and cultural competency, interpersonal relationships, family group dynamics, mental disorders, additions, impacts E. Provide assessments, evaluations, diagnosis and treatment of mental health and emotional needs for children, adolescents and adults. F. Provide intervention/treatment techniques, crisis interventions and/or referrals for outreach services, substance abuse treatment programs, residential treatment facilities and specialty mental services, not contracted with TWP as appropriate. G. Answer referral questions and provide adequate evaluation, diagnosis and treatment recommendations. H. Develop treatment/behavioral plans with clients to enable self management and invest in their own treatment. I. Consult with Psychiatrist and Psychologist and other medical professionals to ensure best treatment outcomes. J. Maintain accurate and detailed record-keeping and ensure all patients receive follow-up. K. Ensure all patients receive follow-up consultation by the integration team. L. Work as a member of a multi-disciplinary setting and provide unscheduled services as needed. M. Educate staff and patients on community resources for effective patient care. N. Travel to all TWP sites and throughout the organization’s service area and the State. O. Work flexible hours, including evenings and weekends as needed. P. Adhere to corporate and departmental policies, practices and procedures. Q.
