TEMPLATE C- Individual Placement Description Northern Deanery Foundation School (NDFS) North Cumbria University Hospitals NHS Trust Individual Placement Description Placement Foundation 2 General Practice The department/Practice FY2 General Practice placements are based in Cumbria and consist of city and rural locations All Practices providing placements have a reputation for delivering high quality care and are high achievers in the Quality and Outcomes framework and deliver a wide range of services including : The type of work to expect and learning opportunities Chronic Disease Management, Health promotion Smoking cessation Sexual health services Minor Ops In house anti-coagulant monitoring Drug and Alcohol workers Counselling services Ante-natal clinics All FY2s undertake a comprehensive Induction Programme which includes spending time with a variety of internal and external services. This is to ensure the F2s are fully familiarised with General Practice prior to their clinical work commencing. The overall objective of the F2 placement in General Practice is to: Take a history and examine a patient Identify and synthesise problems Prescribe safely Keep an accurate and relevant medical record Manage time and clinical priorities effectively Communicate effectively with patients, relatives and colleagues Use evidence, guidelines and audit to benefit patient care Act in a professional manner at all times Cope with ethical and legal issues which occur during the management of patients with general medical problems Educate patients effectively Become life-long learners and teachers. Where the placement is based Clinical Supervisor(s) placement for Various General Practices the General Practitioners The F2 will have allocated weekly clinical supervision time to review all patients seen in the previous week. In addition for the first few weeks of the placement the F2 will have direct support of a supervisor during clinics. Main duties of the placement Typical working placement pattern Diagnosis and management of patients presenting at prebooked appointment slots including referrals to secondary care or other services. Recording of consultations in electronic patient records. The Trust provides a wide range of Clinical support services and most modern investigational methods are now available on site. The hospitals primarily provide services for North Cumbrian residents, but they also offer services to patients in Dumfries and Galloway, the Borders and parts of Northumberland. Nearly all specialities are represented; three are subregional and several others are supra-district in nature. The hospitals provide healthcare for a catchment area of around 300,000.
