JOB DESCRIPTION JOB TITLE: Integrated Health Services Nurse Care Manager SUPERVISOR: Integrated Health Services Program Coordinator SUPERVISES: N/A POSITION SUMMARY: Serve as a member of an Interdisciplinary Team for Integrated Health Services (IHS) and play an active role in designing an integrated care plan for children and their families. The Nurse Care Manager is primarily responsible for facilitating patient care, anticipating patient care needs, individualizing patient care and adjusting or modifying the nursing plan of care as appropriate. The Nurse Care Manager will utilize effective technical skills for the provision of safe and appropriate care for providing integrated care coordination services to high-risk clients. QUALIFICATIONS REQUIRED: Education/Experience: Licensed RN Knowledge and work experience with mental health and child welfare. QUALIFICATIONS DESIRED: N/A ESSENTIAL FUNCTIONS/JOB DUTIES AND RESPONSIBILITIES: DIRECT SERVICE RESPONSIBILITIES: Use available data, such as assessments, participant evaluations, and service observations to identify health and wellness needs to develop individualized care coordination plans. Promote client health and ensure personal health goals are included in care coordination plan. Support ongoing collaboration between physical health and behavioral health providers. Provide health education to clients on preventing and managing chronic conditions. Provide self-management support and development of self-management/relapse plans to assist clients in meeting personal health goals. Collaborate with IHS team to address treatment barriers for high-risk clients. Monitor high-risk care coordination plans and facilitate communication with treating providers as necessary. Educate clients and their families or care advocates in wellness, medication and behavioral health services. Develop symptom response and crisis plans as necessary to ensure more appropriate use of physician visits rather than ER use and hospital admissions. Schedule and coordinate initial, crisis, intake and other appointments with clients. Complete required written reports and documentation according to policy. Provide transportation to appointment when necessary. -1- SS:JobDes/111913/AA Demonstrate flexibility in job duties and schedule in order to serve clients and to assist reaching benchmark standards. Participate in ongoing quality improvement efforts and monitor/evaluate service outcomes. Attend regularly scheduled meetings and training sessions to enhance team performance. Participate in various workgroup and committee activities to support efforts to improve the quality of services. Assist with client enrollment process. Accomplish all tasks as appropriately assigned or requested. NON-ESSENTIAL FUNCTIONS: Other duties, as assigned SUCCESS FACTORS/JOB COMPETENCIES: Knowledge: Customer service skills. Computer proficiency. Strong knowledge base of existing community resources, public and private. Be knowledgeable of word processing software with proficient computer skills, including Microsoft Office, specifically Word and Excel, and be able to type approximately 60 wpm accurately. Possess knowledge of office machines, phone systems and protocol. Skills: Use verbal and written communication to work effectively with co-workers and other professionals in a clear, positive, and productive manner, including writing skills, punctuation and basic grammar. Demonstrate sound judgment, decision making and problem solving skills. Time management skills and ability to be flexible. Confidence to communicate in outreach to other community organizations and personnel. Abilities: Interact professionally with clients and models appropriate behaviors. Prioritize work activities, use time efficiently and develop realistic action plans. Be able to maintain confidentiality with all aspects of information in accordance with practice, State and Federal regulations. Be open to new ideas and accept feedback from others.
