JOB DESCRIPTION Position: Job Coach / Supported Employment Services Service: Supported Employment assists individuals with developmental disabilities in identifying appropriate and satisfying employment, locating and securing that employment, and provides the necessary support to maintain or improve employment. Purpose of position: The primary focus of the Job Coach is: 1. 2. 3. 4. Provide support for people with disabilities to have success in their employment. On the job training and ongoing support. Excellent customer service. Documentation of job coaching support provided. Qualifications / Education: The successful Job Coach is reliable, energetic, professional, flexible, and capable of working both independently and as part of a team, and is a positive role model for Supported Employment’s clientele. Qualifications include: Satisfactory oral and written communication, math, and computer skills. A high school diploma or its equivalent. A minimum of three year’s experience with adults who have developmental disabilities, (or similar experience is strongly preferred). The ability to maintain First Aid and CPR certification. This position may require a California driver’s license, a good driving record, and a registered / insured vehicle. A Department of Justice clearance. Requirements: Personal Assistance: Ability to assist individual in activities of daily living as well as related job tasks. Lifting / Physical Demands: Ability to lift 50 or more pounds frequently. Challenging Behaviors: Ability / willingness to support individuals with challenging behavior. Ability to read and interpret documents, to write specific reports and correspondence, to apply mathematical concepts to practical situations, to solve complex problems, and to interpret written and verbal instructions is required. Reports To: Program Director Status: Hourly position. Part time or full time: this position is eligible for all applicable benefits if regularly scheduled for 30 hours or more each week. Work Schedule: Hours dependent on consumer’s need. Staff must be available during business, evening, weekend, and holiday hours. Duties / Responsibilities: *Supports the development and ongoing revisions to Individual Habilitation Service Plan by providing input and information. *Tracks consumer’s progress towards goals and objectives in IHSP. *Breaks job duties and assignments into specific steps to facilitate consumers learning. *Monitor consumer’s level of performance including determining when support can be faded. *Monitor quality of consumers work to ensure it adheres to industry standards and all tasks are completed within acceptable time frames. *Serves as a liaison between employee and employer and facilitate appropriate worker to supervisor relationship. *Maintain accurate records on job coaching services provided to each consumer. *Document and recommend to Program Director any action necessary to overcome employment barriers. *Maintains current First Aid and CPR certification. *Responds to crisis in accordance to policy and training Develops and maintains written documentation, as requested. Makes all decisions based on agency Vision and Values Statement.
