REQUEST FOR ACCESS TO THE MISO’S
MODEL-ON-DEMAND SOFTWARE
Market Participant
The Midcontinent Independent System Operator, Inc. (“MISO”) and
, acting
as the MOD Security Officer and authorized representative on behalf of
(“Company”), hereby enter into this agreement by which the MISO will provide
access to Model-On-Demand software. As part of this agreement,

Company hereby provide the MISO with a list of individuals who should be granted
access to MISO MOD Base model in the Model-on-Demand application.
Furthermore,

Company wishes to gain access to such information for employees, agents, and
consultants of Company and therefore

agree that he/she, as MOD Security Officer, is duly authorized to execute this
Request on behalf of Company and that this Request, the MISO Universal NonDisclosure and Confidentiality Agreement and the Critical Energy Infrastructure
Information (CEII) Agreement shall bind and be enforceable by and against the
employees, agents, or consultants of Company who are listed below.

in writing of any change to the list below.

(Please note that access to the MISO’s Model-On-Demand software shall be granted
at a single level of Market Participant. This level will allow the user to access the
Model-on-Demand Base case topology.)

Be advised that a six (6) month period of inactivity on the system by a user will
result in the user’s account being de-activated. The account can be reactivated by
contacting Marc Even, (651) 632-8421 or meven@misoenergy.org. Additionally, the
MOD Security Officer is required to certify on an annual basis that all of the
individuals listed should be receiving access to the application.
