ADDENDUM TO LETTER OF OFFER – CLASSIFIED STAFF POSITION
University of Cincinnati
While your signature on the offer letter indicates your intention to accept this position, for
final approval of hire or rehire, you must do the following:

Complete the federal I-9 (via I-9 link) (Employment Eligibility Verification) and
required employment forms described throughout this letter, and submit them, and
all supporting documentation on the date of your scheduled New Employee
Orientation.

Attend and complete New Employee Orientation on the date scheduled.
This is a classified position. In this position you will serve a six-month probationary period.
NEW EMPLOYEE ORIENTATION
You are scheduled to attend New Employee Orientation on (Date). This will be your first
official day of employment and attendance is mandatory for all new staff hires/re-hires.
The session begins at 8:00 am. and ends at approximately 12:30 pm. Orientation is held at
University Hall, Room 450, 51 Goodman Drive, Cincinnati, OH 45219. Free parking is
available in the Kingsgate Garage at the Marriott Hotel next door to University Hall. Upon
entry into the garage, please take a parking ticket and bring it with you to the New
Employee Orientation for validation. Please access the building from Kingsgate Garage
utilizing the elevators marked University Hall.
New Employee Orientation is UC’s way of welcoming you to our community. As a new
employee of the University of Cincinnati, you have a world of opportunities ahead of you.
UC is regarded as one of the top research and teaching universities in the United States, and
we are proud of our accomplishments and plans for the future.
During Orientation, you will learn about UC and meet with fellow new employees. New
Employee Orientation:

Provides you with information that will ease your transition into the workplace

Paints a clear picture of the institution as a whole

Introduces you to university policies, procedures, customs and traditions

Lays a foundation for your career with the university

Helps you to adapt faster to your job and department
If you miss your scheduled New Employee Orientation for any reason, your hiring
department contact will be in touch with you to discuss whether your attendance at a future
New Employee Orientation will be rescheduled. You will be unable to begin employment
with the university until you attend New Employee Orientation. As a result, your
start date will be delayed, and your first pay date and benefits eligibility date may be
delayed as well. Therefore, it is imperative that you attend New Employee
Orientation on the scheduled date.

REQUIRED DOCUMENTS AND INFORMATION FOR ORIENTATION
At New Employee Orientation, you must complete important payroll and employment
verification documents including the I-9 which you will complete on the computer at
Orientation and the Payroll/Personnel Information forms. You may access these forms at
http://www.uc.edu/hr/toolkits/new_hire.html. If you are unable to print the forms, copies
will be provided to you during the New Employee Orientation.
The University of Cincinnati is an E-Verify Employer. The information provided by the
employee on the electronic I-9 (including Social Security Number) will be processed
through the E-Verify system.
Be sure to bring the appropriate identification documents with you to the New
Employee Orientation for the authorization of the I-9. You will not be permitted to
participate in Orientation if you have not brought acceptable compliance documents
with you. The acceptable documents include such original documents as Social
Security card, driver’s license, birth certificate, or any valid combination of such
documents.

Note: You will not be able to attend the New Employee Orientation and begin your first day
of employment with the university until you have:
1. Provided the acceptable, original form(s) of identification and employment
verification.
2. Completed the I-9.
3. Successfully completed a background check, which is a condition of employment at
UC.
BENEFITS PACKAGE
A comprehensive benefits package is one of the many advantages to employment with UC.
You will have an opportunity to learn more about your benefits during New Employee
Orientation.
However, you will want to visit our New Hire website at:
http://www.uc.edu/hr/toolkits/new_hire.html prior to attending Orientation so you are
familiar with your benefits options.
Your position is considered a/an ______________ position. Please refer to the benefits website
at www.uc.edu/hr/benefits to read about UC’s benefit plans. You will want to discuss
available options with your family prior to attending Orientation. You will have time during
the Orientation to enroll in benefits, such as health and life insurance utilizing our online
Employee Self Service (ESS) tool. Human Resources representatives will be available to
assist you. You will also complete federal and state tax withholding enrollments and payroll
bank direct deposit enrollments via Employee Self Service (ESS). Your benefits (default or
otherwise), will be effective on the first day of the month following 28 days of employment.

If you elect to enroll your dependents (spouse, domestic partner, and/or children) in UC’s
medical or dental plans, you are required to provide documentation confirming their
relationship to you. You will be contacted by our vendor HMS Employer Solutions and
asked to provide verification documentation. Information about the required
documentation can be found on the UC benefits website at: www.uc.edu/hr/benefits.
You will find information about how to access ESS on the new hire website. If you need
assistance with your enrollment or have any questions pertaining to your benefits, please
feel free to contact the Human Resources Service Center at 513-556-6381. Remember,
you not are able to access ESS until your hire date.
RETIREMENT PLAN INFORMATION
Employees of Ohio public colleges and universities do not participate in the federal Social
Security system, other than making contributions to Medicare. From your initial date of
hire, you will be enrolled in the Ohio Public Employees Retirement System (OPERS).
As a regularly scheduled, full-time (100% full-time equivalent) employee, you can elect to
change from the state retirement system (OPERS) to the Alternative Retirement Plan (ARP).
You have 120 days from your date of hire to choose the ARP or remain with your assigned
state retirement plan (OPERS). Once you have chosen your retirement plan, your decision
cannot be changed while you are employed by the university.
In order to make your election, you must complete Section I of the Retirement Plan Election
Form available at http://www.uc.edu/hr/toolkits/new_hire.html. You must complete this
form whether you choose to remain in OPERS or elect participation in the Alternative
Retirement Plan. Please submit your form within 120 days of hire to the address shown on
the form.
If you elect OPERS within your 120 day election period and you are not already a member,
you have an additional time period to select between the OPERS Defined Benefit, Defined
Contribution, and Combined Plan options. (Note: If you are a new member of OPERS,
OPERS will send you an enrollment kit for your selection between the Defined Benefit,
Defined Contribution and Combined Plan options). You have a total of 180 days from your
eligible appointment date to select between the (3) OPERS options. If you are already a
member of OPERS, and elect OPERS as your retirement program choice, you will continue to
participate in the OPERS retirement plan in which you are currently enrolled.
DEPARTMENTAL ORIENTATION
After the New Employee Orientation session concludes, you will report to your designated
work area for continued departmental orientation. Your Hiring Manager is aware that you
are attending the New Employee Orientation.
STAFF NEW HIRE CHECKLIST
o
Sign and return a copy of the Offer Letter before (Date), which will indicate your
intention to accept the position. Be sure to keep a copy for yourself.
o
Print and complete the required payroll/personnel forms indicated on page (2)
under REQUIRED DOCUMENTS AND INFORMAITON. Forms will be available at
New Employee Orientation if you are unable to print them.

Bring acceptable, original form(s) of identification documents with you to the New
Employee Orientation, as specified on page (2), under REQUIRED DOCUMENTS
AND INFORMATION.
o
Bring your bank/institution information (routing/transit number and account
number), and tax withholding elections information with you. Please refer to pages
(2) and (3), BENEFIT PACKAGE and RETIREMENT PLAN INFORMAITON.
o
Attend New Employee Orientation on the date indicated on page (1) of this letter.
