All players must attend religious education at either their parish school or their parish religious education program to be eligible.
Jr. Varsity players must be born after December 31, 2002 and in the 6th grade or below during the 2015/2016 school year.
Varsity players must be born after December 31, 2000 and in the 8th grade or below during the 2015/2016 school year.

I, parent/legal guardian, do hereby grant permission for my child, a minor, to participate in the Catholic Grade School
Conference football league. I hereby for myself and for my child as the natural guardian / legal guardian, my heirs,
executors and administrators, waive, release and discharge forever, any hereafter accrues to me against the
Catholic Grade School conference of Chicagoland, their respective agents, officers, representatives, successors and
or assigns for any and all damages, which may be sustained and suffered by my child in connection with my child's
said participation in football and or arising out of my child's traveling to or participating in and returning from
football games or practice and agree to indemnify them and hold them harmless there from.
I also acknowledge that the current rules require that the helmet be NOCSAE approved and I assume the
responsibility of obtaining, examining and maintaining all my child's equipment and the Catholic Grade School
Conference, its officers, agents, servants and directors have no responsibility or liability as to any equipment and I
hereby release them each and all of them from any claim, demand or liability and agree to indemnify them and hold
them harmless there from.
My child has been examined by a physician and is physically able to participate in football and I am not aware of any
physical problems that would prevent my child from participating.
