Prom 2015 Information and Check List
“Enchanted Evening”
When: Friday, May 1st at 7:00 pm– Arrive to PR by 5:00 pm; buses load at 5:15 pm
** Students are to gather in the North gym and parents may assemble in the South gym or
outside. Parents are NOT to be in the hallways. Thank you.
Where: The Stonegate, Hoffman Estates, Illinois
Who: Prairie Ridge Juniors, Seniors and their guests may attend. Anyone attending Prom must
be under the age of 21.
PAYMENT: Tickets may be charged this year. You may pay for your ticket online using this link.
http://chsd155.revtrak.net. Print off the receipt and attach it to your permission slip.
Checks and Cash are still accepted.
Tickets: Tickets will be $75.00 per person. Tickets are sold ONE DAY: Saturday, April 11th:
9:00-12:00 – PR Foyer. Please have one person bring in your whole table’s forms and
payments. Everyone who wants to attend Prom must have a permission slip signed by a parent
or guardian in order to purchase a ticket. The permission slips are available on the PR website.
Non-Prairie Ridge students must attach a copy of a picture ID verifying he/she is UNDER 21
years of age. In addition, he/she must have an administrator’s signature from his/her school and
from Prairie Ridge. There are no refunds on prom tickets for any reason.
Buses: Buses will load in the parking lot off the cafeteria starting at 5:15 P.M. and depart at
5:30 P.M. Buses will leave The Stonegate at 11:00 P.M. and return to Prairie Ridge about 12:00
A.M. Bus assignments will be made when you purchase your ticket. YOU ARE NOT
ALLOWED TO CHANGE YOUR BUS ASSIGNMENT!!
Tables:
Table reservation forms are available on the PR website. To reserve a table with your friends,
complete the table reservation form, writing in the full names of all 12 people. Turn in the table
reservation form with the signed permission forms and payment for all students at your table. All
students will have an assigned table. NOTE: You can still attend prom if you don’t have full table
of 12; we will arrange the seating.

___ Table Form: All Names spelled correctly
___ Permission slips are signed by the Dean, if needed, for each non-PR person at the table
___ Payment: receipt from online payment, checks made to PRHS, or cash
___ Bring all the papers and payment together to tickets sales on April 11th: 9:00-12:00 in the
PR in the foyer. Parents may also bring in the table’s forms and payments. All students do not
need to be present to purchase the table’s tickets.
