Introduction During the inspection of the project the staff of Independent Engineer and Concessionaire’s representative were present. During inspection and site visit, the Concessionaire’s representative & Independent Engineer’s key personnel were discussed in meeting on various issues as mentioned below. Concessionaire Progress Work Progress:The concessionaire has already started the activities at Durgapur section, Asansol section and Raniganj section and Dhanbad section for C & G, Embankment, Sub-grade, GSB, WMM, widening of pipe culverts, Construction of box culverts and Vehicular under passes, Grade separator, Minor bride & Major Bridges. The concessionaire is being advised that to short out the problem the shortage of materials (GSB, WMM and DBM aggregates) and increase the resources and plan & Machinery should be mobilized fully and put together to give full momentum to the progress of the project highway with proper methodology and well planned sequence of operation should be used to have better results and desired rate of progress of the project. GFC drawing issued GFC drawing issued GFC drawing issued 7 Inspection report for the month of JANUARY 2015 INDEPENDENT ENGINEER OBSERVATIONS ON THE STATUS OF WORK & PROJECT ISSUES A. PROJECT ISSUE:1. Progress of work to be ensured as per Project Milestones of Schedule-“G” of Concession Agreement. 2. Concessionaire yet to mobilize fully for Construction of Project Highway in all Section. 3. DW&SD - of Jharkhand State (Km. 405+350 to Km. 436+700):- NHAI has recommended to the competent authority the modification to project plan, profile/features from Km. 405+350 to Km. 436+700. a. IE commented on concessionaire proposal and compliance awaited from concessionaire side. 4. Godindpur and Nirsa (Govindpur : Km. 406+460 to Km. 509+580):- During the meeting convened by the Deputy Concessionaire, Dhanbad on 10.06.2014, it was proposed to change the alignment by adopting 45 m ROW, in both the towns i.e. Govindpur and Nirsa with dense dwellings/shops. a. IE commented and under review (Govindpur section). B. QUALITY CONTROL:1. Documentations of Quality Control to be maintained in Lab, Structure-wise for Structure and Chainage-wise (Km-wise) of Road Work for each Activity/Item Separately for the executed work. 2. “Material” to be used only from Approved Source as per Section 100 Clause 117 of MORT&H Specifications. 3. Stock Register for Material like Cement, Admixture, steel, Bitumen and other major material to be maintained properly in Lab. 4. Third Party Test of Materials i.e, Cement, Steel, Bitumen, Emulsion, Admixture etc are to be conducted Frequency Wise as per IS/IRC and record to be kept in order. C. PROGRESS OF WORK:1. Concessionaire has also advised to start the activities at all available front. In km 441.400 to km 469.000 all available fronts of structure activities should be immediately start. 2. Work to be expedited, so that the progress as per asking rate in order to maintain for the completion of the project as per schedule time. 8 Inspection report for the month of JANUARY 2015 3. There is shortage of GSB/WMM at Apco section –II and DBM aggregates problem in all three sections. Concessionaire has advised to arrange the above materials from his level to increase the progress of work and achieve the required target. 4. In adequate curing arrangement has been observed at structures site. The same should be immediate improved for proper curing of structures as per required for minimum periods. 5. Joints of shuttering plates of structures are not smooth, concessionaire has been instructed to repair immediately and proper care should be taken during shuttering. 6. Fly ash embankment construction in Panagarh bypass portion at some locations are going on, but proper methodology has not been adopted, confinement layer has not been provided at edge of fly ash. Concessionaire has been instructed to strictly follow the proper methodology of fly ash embankment construction. 7. Fly ash has been dumped in pond section without removal of muck & sludge. Concessionaire has been instructed to remove the dumped fly ash from that section and re do the work only after complete removal of muck & sludge from pond. 8. Oversized lumps have been observed in embankment soil at Panagarh bypass portion. Concessionaire has been instructed to oversized materials should be broken in specified shape & size before laying and grading. 9. Earth material is being used as backfill at the culvert at km501+260(RHS), which is not acceptable. Concessionaire has instructed to remove the said material and replenish the same with suitable approved material. 10. It has been observed that laying of DBM has been done from km 471+600 to Km 472+810(LHS), but the Mix Design of same has not been submitted for IE review . Concessionaire has been instructed to submit the MIX Design of DBM immediately for our review. SAFETY & MAINTENANCE OF EXISTING ROAD:1. The overall safety measures during construction at site were inspected / reviewed. The concessionaire has provided the safety arrangement as per the provisions of concession agreement are lagging and the concessionaire are being reminded regularly to improve safety measures at work site. 2. The concessionaire was requested to ensure safety measures by keeping a separate safety team headed by a qualified safety officer for monitoring day to day safety arrangement all along the project corridor by providing highway traffic management system. 3. Concessionaire was also advised to ensure the safety of workers by providing PPE (personal protective equipments) to the workers/ labour during execution. 4. Advance marking sign regarding portion of any of the highways damaged/closed or diversion or any other information which needs to be advised to the road users should be prepared in advance and kept ready to enable immediate necessary action in the matter to be able to ensure hassle free flow of traffic on existing highway. 5. Unauthorized median opening should be immediate closed and median opening signage should be provided at authorized median opening locations. 6. Concessionaire has requested to please make proper safety arrangement during cutting of trees, logging and transportation. 7. Object marker signage to be immediate provided at toll plaza (km. 507 & km 454) and all median opening locations. 9. 9. Signage, Retro Reflective Tapes, Warning Sign should be provided at construction zone portions and also arrangement of traffic during construction and maintenance shall confirm to the requirements of IRC SP: 55, clause 112 of MORT&H Specification and provisions of concession agreement. 10. The Maintenance of existing highway is being done by concessionaire. Repairing of pot holes works on the project stretch has been considerably nil in this month. 11. Some pot holes are observed at different location i.e. Raniganj section, Bhirangi section & Panagarh section, which needs immediate attention to repair and keep maintained. 12. Trimming of over growth of plants and shrubs in median portion activity started, and advised to complete the trimming activity as early as possible. Median plantation should be maintained properly and re-plantation also should be done properly where plant is damage. 13. Maintenance of existing shoulder of project highway has to be taken up immediately. D. HINDRANCES AND OBSTRUCTIONS:1. Acquisition of Balance Land 2. Demolition of Structures. 3. Shifting of Utilities i.e. Water line/PHE, Electric poles and towers, Gas pipe lines, Petroleum pipe lines. Presently Electrical utility work is in Progress but needs to be expedited for Completion of Project Highway on schedule date. 4. Cutting and Removal of balance Trees. 5. Relocation of Religious Places. 10
