REQUIREMENTS for ENROLLMENT:

Birth Certificate: Bring an original copy (not hospital memento) for all students verifying they will be 3 or 4 years of age by September 1, 2016. A copy will be made with the original returned to you. This is a State of Illinois requirement.

Physical and Dental Exam: General health form, with complete immunizations, signed by doctor must be on file in the school office. Health exams are required by the State of Illinois. Dental exams are not required but strongly suggested.

Students must be able to use the bathroom independently.

All required fees and forms must be filled out completely, legibly, and signed where necessary.
TUITION & REGISTRATION FEE:

Tuition is paid through SMART Tuition on a monthly, quarterly, or bi-annual basis. Tuition that is paid-in-full by
September 1 st will receive a 3% discount. The Registration Fee is due upon application.
Children learn through love, discipline, exploration, participation, communication, trial and error and Christian example. St.
John’s Lutheran is ‘Nationally’ accredited, as well as a ‘State if Illinois’ recognized Kindergarten through Eighth Grade elementary school.
