2.1

History and Rationale: Provide brief history of the academic program at the institution, when it began, its departmental and school/college home, and enrollment history. Explain why the institution is pursuing this extension of the program to a remote site and the market or student demand data that suggests it is needed.

2.2 Relation to Mission: Describe how this initiative is congruent with the mission, strategic goals, and 5-year Academic Plan of the institution.

2.3 Educational Objectives, Assessment and Evaluation:

Provide the program’s goals for students learning.

Describe how the institution will evaluate the educational effectiveness of the program and assess program goals for student learning,

2.4 Comparable Programs: Indicate what comparable UW System and private programs are available and their geographic proximity to the location of the proposed remote delivery site.

2.5 Facility: Identify the specific location and describe the facility in which this program will be offered. Indicate how the appropriate classrooms, offices, laboratory facilities (if required) and technology will be provided at the remote site. Include information on ADA compliance of the facility.

2.6 Delivery Method: Indicate whether the program will be face-to-face, hybrid, or online and the specific means of delivery, e.g., lecture, lab, video streaming, asynchronous, web-based, etc.

2.7 Diversity: Describe how extending this program to a remote site will serve the goals of diversity and inclusion and enhance participation by traditionally underrepresented students, faculty, and staff.

Financial Considerations: Describe how this program will be financed. If the tuition/cost for this program differs from the regular institutional undergraduate or graduate tuition, indicate how tuition/fees will be set and which UW System financial and administrative policies apply (e.g., differential tuition).

3.2 Commitment to Maintain Program: Describe any academic, financial, technical, and contractual agreements in place to sustain the program over time.

4. STUDENTS

4.1 Prospective Students : Describe the audience for this remote site delivery program.

Explain why these students cannot be served through the existing program or other programs.

4.2 Enrollment: Indicate how many students will be enrolled in the program initially at the remote location and how many when the program is at capacity. Also indicate how many graduates per year are expected once the program is fully enrolled, and how that is projected to take.

4.3 Access for Individuals with Disabilities: Discuss the methods by which you will address the needs of individuals with disabilities who attend the remote site.

4.4 Access to Academic Support and Student Services: Describe how the institution will provide adequate access to library services, computing and technical support, admissions, financial aid, orientation to the program, academic and career advising, and health and counseling at the remote site. How will student segregated fees be assessed?

5. PERSONNEL

5.1 Faculty/Instruction Academic Staff: Indicate the number of FTE faculty, academic staff, and classified staff that are needed to offer the program at the remote site and where they will be located.

5.2 Support: Describe how the program will provide faculty with administrative, technical, or other needed support in order to teach at this remote site.
