APPENDIX 4 APPOINTMENT OF A NEW LEO LECTURER III Last Updated August 2015 Pool Report Guidelines 1. Complete the top section of the LSA Pool Report. Be sure to include the LSA position number AND the job requisition number. The job requisition number is obtained from eRecruit when the faculty position is posted. Both numbers are required on the LSA Pool Report. Positions for which a waiver of posting has been obtained (i.e., person specific hires) should still have a completed LSA Pool Report. Both the waiver and the approved LSA Pool Report should ultimately be submitted with the dossier for EC approval and to Deb Erskine with the appointment paperwork. 2. Candidates Selected for Campus Visit - Complete this information including name, ethnicity, and gender (where known) for all candidates brought to campus for consideration. 3. Other Candidates Selected for Off-Campus or Telephone Interview Only - Complete this information including name, ethnicity, and gender (where known) for all candidates interviewed without a campus visit. 4. Summary of Applicant Responses to Gender/Ethnicity Survey - Complete gender and ethnicity information based only on those faculty applicants who respond to the LSA gender/ethnicity survey. a. Send an email to facsurvey@umich.edu to obtain URL for survey. One of the Associate Dean’s Executive Secretaries will e-mail you the URL link once the survey has been created to be included in your communication to all the applicants. b. Send e-mail reply to all faculty applicants acknowledging receipt of their application materials and inviting them to complete the voluntary gender/ethnicity survey. Include the URL in the communication. c. Once the unit decides which candidates they would like to bring to campus for an interview, send an email to facsurvey@umich.edu and request a summary report of the survey responses. It is important to include the job requisition # and your unit’s name when requesting the survey results. One of the Associate Dean’s Executive Secretaries will e-mail the survey summary to the unit for completion of page 2 of the LSA Pool Report. d. Using the survey summary report, complete the Summary of All Applicant Responses to Gender/Ethnicity Survey section on page 2 of the LSA Pool Report. e. Anticipated turnaround from the Dean’s Office is ½ working day or less for steps a. and c. above. 5. Summary of All Applicants – Complete this section based on the total number of applicants who applied for the position. Candidates who did not respond to the survey should be categorized as “unknown”. 6. Other Information about the Search – Complete the information regarding gender and ethnicity of the search committee and the name and contact information for the search committee chair. 7. Submit the completed LSA Pool Report via e-mail to the Associate Dean’s Executive Secretary for review and approval by the Associate Dean prior to arranging for any campus visits.
