Southview High School National Honor Society Candidate Form Checklist Please do NOT include this checklist as part of the candidate form packet. In order to be eligible for full candidacy into National Honor Society, each candidate must submit candidate forms to Faculty Council. Candidate forms may be obtained from Southview’s homepage using the link to National Honor Society. The goal of these forms is to provide documentation verifying National Honor Society’s four pillars: Character, Scholarship, Service and Leadership. Please submit the following: 1) Candidate Signature Page: This page must be signed by both the candidate and one of his/her parents or guardians. 2) Leadership Documentation: a) Southview in-school leadership activities: Type all of your Southview in-school activities, your role for those activities, any leadership positions, and the year/grade you were involved. For verification purposes, an advisor name and initials are required for each activity. b) Southview out-of-school activities: Type all of your out-of-school activities, your role for those activities, any leadership positions, and the year/grade you were involved. For verification purposes, an advisor or supervisor must submit a brief letter or email on the letterhead stationary of their organization confirming your participation. c) Leadership essay: One of the criteria for admission to National Honor Society is leadership. Please write an essay answering, “What makes you a leader?” These experiences need not be confined to those completed at Southview. Possible topics to discuss could include, “What do you do in class to exemplify leadership?” Or, “In leadership positions held, how have you shown leadership?” Your essay must be typed and professional looking, with no misspellings and/or grammatical errors. d) Verification letter (this form can be used to request a verification letter; this is not the actual verification letter) 3) Service Documentation: a) Southview in-school service: Type all of your Southview in-school service activities, the dates or year/grade in which you participated and a detailed explanation of what the service activity entailed. Include only those activities where service was the primary goal. For verification purposes, an advisor name and initials are required for each activity. b) Southview out-of-school service: Type all of your Southview out-of-school service activities, the dates or year/grade in which you participated and a detailed explanation of what the service activity entailed. Include only those activities where service was the primary goal. For verification purposes, an advisor or supervisor must submit a brief letter or email on the letterhead stationary of their organization confirming your participation. c) Verification letter (this form can be used to request a verification letter; this is not the actual verification letter) 4) Additional Information: The candidate may attach an additional sheet detailing any other pertinent information. For example, the need for work might have resulted in a lower amount of activities. Use this to explain the type of job currently held, the amount of time spent on the job, and the reason for the work. In the case of disciplinary concerns such as multiple detentions or a serious infraction from previous years, explain what happened and what has been done to overcome this deficiency. 5) Personal Information Form: This form includes how you wish to have you name on the NHS certificate should you be invited for membership. 6. Faculty and Staff Recommendation: For Character and Leadership, communication between the National Honor Society Advisor and the Southview Faculty and Staff occurs. For this part of the Candidate Form, you do not need to submit anything.
