APPENDIX 5 FIFE LOCAL IMPROVEMENT TARGETS 2005-08 Introduction: As part of recent Spending Review Ministers announced three national targets: Reduce the number of people waiting to be discharged from hospital into a more appropriate care setting by 20% per annum between 2005 and the end of 2008, cutting to a minimum the number of people waiting more than 6 weeks to be discharged. Increase by 2008 the number of older people receiving intensive home care to 30% of all people receiving long-term care. By 2008/9 reduce the proportion of older people (aged 65+) who are admitted as an emergency inpatient two or more times in a single year by 20% compared with 2004/5, to release capacity in hospitals and improve patient care. Community Health Partnerships will target those most at risk in the community, particularly patients with multiple long term conditions, with monitoring, support and prevention, using appropriate methodologies such as intensive case management and disease management. The Local Improvement Targets outlined in this report demonstrate how the partnership in Fife will work towards meeting the longer term national targets. Fife Local Improvement Targets National Outcome Baseline 2004/05 Target set for 2005/06 Measures Supporting more people at home as an alternative to residential or nursing home care SMART technology equipment in houses for 28 service users in Fife – includes equipment to monitor temperature, movement, falls. Increase provision of SMART technology equipment in houses to enable 50 service users to remain in their own homes Increased number of service users enabled to continue to live in their own homes In 2004/05 the average proportion of people receiving over 10 hours was 10% of the total. Increase proportion of people receiving home care over 10 hours per week (all client groups) to 12% of all people receiving home care Support 20 Home Carers through the Modern Apprentice Scheme Increased proportion of people receiving over 10 hours of home care per week. Reduce the length of time from initial referral to first service start to average of 14 days (all community care referrals). Currently 110 NHS staff are trained to use the Extended Liaison Policy. 76 Hospital OTs can order Social Work funded equipment. In addition, 35 NHS support staff have been trained in fitting and check visiting. Provide annual refresher training for 130 NHS and Social Work OT staff. Number of staff receiving joint training. In the past year, 20 Social Work OTs have received joint training on ELP with NHS colleagues. Joint training facilitates the building of relationships, development of shared values, common understanding of problems and restrictions and possible solutions and improvements. On average, in 2004/05 there were 3 delayed discharges per month because the people were awaiting equipment before being able to return home. Increase the level of joint training to include all SW OTs (36 FTE) Reduce the average number of delayed discharges because people are awaiting equipment to 2 people per month before they could return home. On April 15 2004 there were 121 people in total who were delayed from discharge from hospital. National target for April 15 2005 is 97. Meet national target Number of delayed discharges against national targets Develop a real-time, transparent and accessible shared information system to manage delayed discharge. Aspire to increase the level of satisfaction of service users with the process of single shared assessment close to or to 100% Shared Information System in place. National target to reduce waiting times for patients ready for discharge to maximum of 6 weeks. Target for Fife is maximum of 55 people waiting ore than 6 weeks on April 15 2005. Manual systems are in place in Health and Social Work but these are not linked. Single Shared Assessment: ensuring people receive an improved quality of care through faster access to service and better quality service The average satisfaction rate with the assessment process was 95.7% in 2004/05 Number of questionnaires returned.
