ASSOCIATE DEGREE NURSING PROGRAM West Hills Community College Academic Summary Form Information Sheet APPLICATION PERIOD: (March 2, 2009-April 17, 2009) PLEASE READ ENTIRE INFORMATION SHEET BEFORE COMPLETING THE ASF (ACADEMIC SUMMARY FORM). The Assessment Technologies Institute Test of Essential Academic Skills (ATI-TEAS) will be offered upon preliminary acceptance into the program and student must pass with a 67% to be eligible for the WHC nursing program. Testing will be at the expense of WHC. Testing times and dates will be announced after the applications are evaluated. The Selection Process will be based on a point value system. The criteria is listed below. In the event of a point tie for the available seats, a combination of TEAS score and randomized selection will be used for the final selection of the candidates. All transcripts and supplemental documents must be included or they will be considered incomplete and processing will be terminated. All qualified packets received before the stated deadline will be treated equally regardless of the submission date. A completed packet should contain the following documents: Completed ASF form. Please list all colleges attended on the ASF including West Hills College Lemoore and all affiliated campuses. Please note: failure to list all colleges previously attended may result in your application being denied. Proof of current Health Care experience (resume…) Submit Two (2) official, up to date, sealed transcripts from all colleges attended (including West Hills College Lemoore and West Hills College Coalinga). Please request that the transcripts be mailed to you first, then forward the packet as a whole to the Health Careers office. DO NOT SEND TRANSCRIPTS SEPARATELY. TRANSCRIPTS MUST ACCOMPANY THE ASF FORM. (When requesting transcripts, please obtain a copy for your records so you will be aware 02/23/09 of the content.) Please note that it may take 4-6 weeks to obtain transcripts, so do not wait until the last minute. One unofficial copy of your high school transcripts or copy of diploma or GED certificate. Catalog course descriptions – transcripts from colleges other than those affiliated with West Hills Community College District (specifically, West Hills College Coalinga, West Hills College Lemoore, North District Center) or CSUF must be accompanied by a catalog course description for each course completed. Foreign Records Evaluation – All students who have completed any course outside of the United States will need to complete a foreign record evaluation if any of the courses will be used towards graduation. All foreign educational records must be evaluated by an official accredited educational document service, (see below). Please request that the service provide an official sealed report to be submitted with the program packet. The report should include U.S. educational equivalency for terms, degrees, course listings, and semester credit hour. Most agencies classify this as a “course by course report”. VERY IMPORTANT! It is advised that students with foreign education meet with a counselor to determine if courses will be accepted for application requirements. Completion of prerequisites with a minimum of C or better in each course and overall GPA of 2.5 and Prerequisite cumulative GPA of 2.5, as well. “In Progress” courses are not accepted. Grade points and units will be tallied from all college work attempted. If a course is repeated at another college for a higher grade please, include an attached note indicating the course that has been repeated and where the course was taken. Please refer to the Nursing Counselor for any concerns regarding your cumulative GPA. COMPLETED APPLICATIONS may be delivered in person or by regular mail; however, receipt cannot be guaranteed. Strict adherence will be paid to the application deadline. Allow 6-10 weeks after deadline for review of documentation. Please do not call the office to find out the status of your application – all applicants will be notified of their status by mail. You may contact our office to inquire about receipt of ASF if submitted by mail. Students selected to participate in the Associate Degree Nursing Program must attend a mandatory orientation for the program. It is highly recommended that a family member also attend.
