Class 12: Emergency Diseases; syncope, sunstroke, malaria, convulsions, high fever, collapse, drowning, coma Xianhui Li Syncope • Syncope is manifested by sudden fainting, pallor, cold limbs and loss of consciousness. Etiology and pathology • Debilitation and overstraining, profuse bleeding or emotional disturbance results in exhaustion of qi or rebellious qi disturb the mind. Deficient syndrome • Manifestations: – Feeble breathing, spontaneous sweating, pallor, cold limbs, deep and thready pulse. • Treatment principle – Reinforce qi, invigorate yang and promote resuscitation • Acupuncture Treatment DU26, DU20, PC6, Ren6, ST36 – DU20 and DU26 are selected to regulate qi of the governor vessel, effecting resuscitation – PC6 is the point for resuscitation – Ren6 and ST36 are good for reinforcing qi and invigorating yang • Herbal treatment: du shen tang or shen fu tang Excess syndrome • Manifestations: – Coarse breathing, rigid limbs, clench jaws, deep and tight pulse. • Treatment principle – Regulate the flow of qi and promote resuscitation • Acupuncture Treatment: DU26, LI4, PC9 zhong chong, PC8 lao gong, LIV3, KID1 – DU26 and PC9 are used to promote resuscitation – LI4 and LV3 are the points for regulating the circulation of qi and blood – PC8 and KI1 promote a clear mind and smooth flow of qi and blood • Herbal treatment: si mo tang Sunstroke • Sunstroke is an acute case occurring in summer, manifested by high fever, irritability, nausea, or even followed by collapse and loss of consciousness. Etiology and pathology • Invasion of summer heat consumes primary qi and the body fluids, penetrates the pericardium and disturbs the mind. Mild type • Manifestations – Headache, dizziness, profuse sweating, hot skin, coarse breathing, dry mouth and tongue, dire thirst, superficial, large and rapid pulse. • Treatment principle – Eliminate summer heat • Acupuncture Treatment DU14, PC6, LI11, BL40 – Du14 the meeting point of the governor vessel and all yang meridians, BL40 and LI11 are important point for eliminating heat, are used to dispel the summer heat – PC6, luo point of hand jue yin, is chosen to reduce the fire and to protect the heart • Herbal treatment: bai hu jia ren shen tang Severe type • Manifestations – Headache, dizziness, profuse sweating, dire thirst, shortness of breath at first, and then collapse, loss of consciousness, sweating, deep and forceless pulse. • Treatment principle – Eliminate summer heat and promote resuscitation • Acupuncture Treatment DU26, DU20, Shixuan, PC3, BL40 – Summer heat is a pathogenic factor of yang nature, which is apt to attack the pericardium and disturb the mind. – Du26 and DU20 are selected to promote resuscitation. – PC3, the He sea point of hand jue yin, BL40, the He-sea point of foot tai yang, remove the heat from the blood – Blood letting at shixuan can reduce heat and promote resuscitation • Herbal treatment: qing shu yi qi tang, an gong niu huang wan Malaria • Malaria is a disease characterized by paroxysms of shivering chills and high fever occurring at regular intervals, mostly found in late summer and early autumn. Etiology and pathology • Invasion of malarial parasite • Pathogenic factors resides in the portion between the exterior and interior and moves outward and inward between Ying and Wei. When it moves inward to struggle with yin, there are chills, and when they move outward to fight with yang, there is fever. Struggle between and antipathogenic factors and pathogenic factors causes paroxysmal chills and fever. Sticky and yellow tongue coating, wiry and rapid pulse. Splenomegaly. DU14, the meeting point of the three yang meridans and the governor vessel, can promote the circulation of qi in the yang meridians and help to eliminate pathogenic factors, in combination with DU13, which can remove obstruction from the governor vessel and regulate yin and yang, they are the chief points for the malaria TE2 and GB41 are two points along the shao yang meridians can harmonize qi of the shao yang meridians. SI3, a point of taiyang meridians of hand, can activate the circulation of qi in the taiyang meridian and the governor vessel and drive pathogenic factors out. PC5, a point of jue yin meridian of hand, is an empirical point for malaria. LI11, a point of yangming meridian of hand, combined with DU14 can dispel heat LR13, the influential point dominating zang organs, can regulate qi in the zang organs Pigen, an extra point, treats the mass in the hypochondriac region Herbal treatment: chai hu jie nue yin Collapse • Collapse is a critical condition characterized by pale complexion, coma, cold limbs, perspiration, and dropping of blood pressure. Etiology • Profuse of loss of blood, severe vomiting or diarrhea, emotional disturbance and chronic disease consume Qi and blood, Yin and Yang, giving rise to separation of Yin and Yang. Differentiation and Treatment • Yang depletion – Weak breath, cyanosis, cold limbs, pale swollen tongue, thin weak pulse • Yin depletion – Irritability, thirst, dry and red lip and tongue, thin rapid pulse. • Treatment principle – Rescue Yang and consolidate collapse • Acupuncture points DU25, DU26, PC6, Moxibustion at DU20, Ren8, Ren4, Ren6 – DU25, DU26 and DU20 are selected to regulate qi of governor vessel, effecting resuscitation. – PC6, luo point of hand jue yin, is chosen to reduce the fire and to protect the heart and calm the spirit – Ren8, Ren6 and Ren4 are located on the lower abdomen along the Ren vessel and are the main points effective for collapse.
