The above named student(s) has/have my permission to join SAINT BRIGID OF KILDARE SKI CLUB and
participate in the six (6) ski trips to Mad River Mountain in Valley Hi, Ohio. I understand that skiing is a
recreational activity and does pose some risk of severe bodily injury or death due to adverse weather
conditions, terrain, and other skiers, among other things. In consideration for the right to participate in
SAINT BRIGID OF KILDARE SKI CLIB, in the event of injury or damage to the above named student(s), I
agree to waive any right of recovery against St. Brigid of Kildare Elementary School, St. Brigid of Kildare
Church, and any adult chaperone, whether or not St. Brigid of Kildare Elementary School, St. Brigid of
Kildare Church, and any other adult chaperone were negligent in any way. I also understand that the
above student may be removed from the SAINT BRIGID OF KILDARE SKI CLUB for any violation of any St.
Brigid of Kildare Elementary School rule. I also understand that any refund of lift passes, lessons, and/or
equipment rental is determined solely by Mad River Mountain. I also understand that there can be no
refund of the transportation charge in any event.
I have read and understand this agreement and I am aware that by signing this agreement, I may be
waiving certain legal rights, including the right to sue Saint Brigid of Kildare Elementary School/Church
and/or any adult chaperone.
