PURPOSE OF MEDICATION
Expected Pharmacological Action
Produces analgesic and anti-inflammatory effect by inhibiting
prostaglandins. May relieve fever through action in
hypothalamus. In low doses intereferes with clotting by
keeping a platelet aggregation substance to form.
Therapeutic Use
Osteoarthritis, mild pain, fever, suspected MI
Complications
CNS: Cerebral edema, coma, seizures, intracranial or subdural bleed.
EENT: tinnitus, hearing loss GI: Bleeding, dyspepsia, pancreatitis GU
ante/post partum bleeding HEMO leukopenia, thrombocytopenia, DIC
HEP: hepatitis MET: hyperkalemia, metabolic acidosis OTHER Reyes
syndrome
Contraindications/Precautions
Bleeding disorders, NSAID sensitivities, GI/Renal/hepatic
dysfunction, acid-base or electrolyte imbalance, 3rd
trimester or breastfeeding
Interactions
DRUG-DRUG: ACE inhibitors, beta blockers,
corticosteroids, diuretics, heparin, ibuprofen, other
NSAIDS, oral antibiotics
DRUG-FOOD: Caffiene, ETOH
Medication Administration
Routes: PO or PR
caps: 162.5mg, 325 mg
tabs: 81mg, 325mg, 500mg
May be enteric-coated to ext. release
supp: 60mg, 120mg, 200 mg, 300mg,
600mg
Nursing Interventions
Enteric coated is not suitable for
pain relief - slowly absorbed.
PR-be alert to bleeding /
irritation. Febrile and
dehydrated will develop toxicity
rapidly. Stop 5-7 days before
procedures. Check LFTs, BUN,
Cr, K, WBC, Platelets
Client Education
Evaluation of Medication Effectiveness
Decrease NA diet
Take with food
Do not crush if enteric coated
Depending on purpose: Reduction in mild pain or fever.
Maybe used for therapeutic anticoagulation (check PT,
PTT, INR)
