Nursing Interventions
Internal and superficial bleeding stroke
Monitor for S&S of excessive
bleeding, neuro checks, no invasive
procedures, lab tests and frequent
vitals
Contraindications
Hypersensitivity to alteplase, active internal bleeding,
history of CVA, recent intracranial or interspinal surgery or
trauma. Severe liver or renal dysfunction
Client Education
Sudden headache, bloody or tarry
stool, any oozing or bleeding from
injection/insertion sites. Remain
quiet and on bedrest while receiving
this medication

Medication Administration
Reconstitute the 50mg vial using a large bore needle (18
gauge), dilute contents with sterile water. Give 0.9% over
60 min with 10% of total dose as initial bolus over one
minute.
