ABC Camp Inspected on May 10, 2014 SUMMARY Copper Co. Ltd. occupies the first floor of a five storied building located in Chittagong Export Processing zone (CEPZ), the floor is rented from BEPZA (Bangladesh Export Processing Authority). The building was constructed in 2012 and production began in the same year. At the time of survey the factory had 250 workers. The Factory was surveyed for electrical safety by Woosun Energy and Construction Co., Ltd. (WEC). The purpose of the survey was to identify significant electrical safety issues and to provide recommendations for remediation based on applicable standards specified by the Accord. The scope of this initial electrical safety inspection was limited to the review and identification of major electrical safety issues. The inspection did not include identification of minor deficiencies, which will be further addressed as part of follow-up inspections. Table below summarizes the major electrical safety issues identified during the inspection. Recommendations have been provided to address each issue. An implementation schedule shall be developed by the factory to remediate each of the findings. The specific timing of improvements, including any requested extensions due to design / installation constraints, shall be submitted to the Accord for approval. Copper Co. Ltd. Page 2 of 6 FINDINGS AND RECOMMENDATIONS Finding #: E- 1 Category: SWITCH BOARD & PANEL Finding: The rubber mats in front of the panels and distribution boards are inadequate in size and in some places, it is missing. Recommendation: Electrically graded insulating mats of required size must be provided in front of all panels that required to be operated on regular basis. Remediation Timeframe: Copper Co. Ltd. Support cables in cable trays. Cable trench with cover may ease the congestion of cables behind the panel(s). Remediation Timeframe: Cables not supported Within 3 months Finding #: E- 3 Category: SWITCH BOARD & PANEL Finding: Temporary connections tapped from the bus through the front side of panel baring to close the panels. Recommendation: Temporary wirings connected to the bus bars inside panel passing through the front door (door kept open) must be removed. Remediation Timeframe: Copper Co. Ltd. Temporary joints Within 1 months page 4 of 6 Finding #: E- 4 Category: SWITCH BOARD & PANEL Finding: Open live conductors not insulated properly and exposed in storage godown Recommendation: Open unused live cables must be removed if not in use, and be well insulated in use Remediation Timeframe: Open live conductor/wire in warehouse Within 3 months Finding #: E- 5 Category: SWITCH BOARD & PANEL Finding: Multiple wires installed in single lug/terminal. Recommendation: Terminate each cable proving individual lug according to the cable size. Multiple cables shall not be terminated on a single point of the bus-bar. Remediation Timeframe: Multiple looping Within 1 month Finding #: E- 6 Category: SWITCH BOARD & PANEL Finding: Multiple wires installed in single lug/terminal. Recommendation: Every wire terminating must be installed using independent lug/terminal. Existing bus bars may be weakened when bunched wires in a terminal is separated. It is recommended to install separate panel or extend / replace the existing bus bars. Remediation Timeframe: Copper Co. Ltd.
