A.
Call to Order: Valerie Wagner called the meeting to order at 2:05 p.m.
1.
Minutes: Minutes of February 22, 2012 were approved with one
correction.
2.
Workers’ Compensation: Valerie Wagner reported that there were two
injuries for the month February. One injury resulted in a total of three days
off from work.
3.
Safety and Health Office: Valerie Wagner reported that Rocky Bonura
and she will meet in April with Dwayne Hayden in Audio Visual to set-up
on-line safety training for those employees who do not have access to a
computer such as those in Facilities.
4.
Facilities Work Order Report: Michael Blada reported that with regard
to work order number 44868, some of the non-skid strips have been
replaced and we are in the process of completing this work order to the
east-side stairway of Natural Sciences.
5.
Old Business:
Natural Sciences – Christie Killduff reported that the emergency lighting
has been installed in the Planetarium.
Christie Killduff also reported that she met with Rocky Bonura regarding
the slip and fall hazard in the NATS building and it was Rocky Bonura’s
suggestion to purchase a non-skid mat to be permanently placed from
outside the restrooms to the Earth Sciences exit doors on the 3rd Floor.
Christie Killduff also reported that the electricians have replaced the
electrical cords on the microscopes.
6.
Division Safety Reports:
Art Department – Adrian Amjadi reported that he will begin completing
the safety reports on the first of each month.

Adrian Amjadi also reported that the concerns that we had in the past with
the ceramics loading dock having to do with excess materials blocking of
the doors; most of the materials have been removed.
Adrian Amjadi stated that he was approached by one of the Teacher’s
Aides in a jewelry class and they were inquiring if the District still has the
service that issues respirators to employees. Adrian Amjadi also stated
that he has respirators in stock to issue to the employees. Valerie Wagner
reported that there is a process that employees must first follow before a
respirator can be issued. Employees must first complete an annual
mandatory respirator questionnaire. The completed questionnaires are
then submitted to our Medical Director for his review and approval. If the
medical director does not approve the questionnaires, the employees must
have a pulmonary exam to make sure that the employee or employees are
fit to use a respirator before a fit test can be completed. Valerie Wagner
recommended that Adrian Amjadi provide her with a list of employees
who will need to be fit tested.
Financial Aide Office:
Elizabeth Fernandez stated that her department is in need of replenishing
of the first aide kits and would like to know if the Office of Safety and
Health would be able to purchase the supplies. Valerie Wagner stated that
she would find out from Rocky Bonura if this could be done.
Elizabeth Fernandez reported that she witnessed one of the palms fell off a
tree and hit an employee’s parked car so hard that I thought two cars had
collided. More likely if the palm would have hit someone, I am sure that it
would have injured them.
Natural Science:
Christie Killduff stated that she had first submitted a work order to repair
the ceiling tiles that have fallen off then I discovered that there was a blue
liquid dripping from the pipes and contacted Debbie Turano and informed
her that the work order should be submitted to HVAC instead due to the
leak. Michael Blada explained that anytime you see blue water leaking
from the pipes this has to do with the valves. When the pressure on the air
conditioning system goes up in the line then there will leaking of blue
water to release that pressure. Either the valves need to be replaced of the
valves need to be tightened in order to prevent blue water from leaking
from the valves. Michael Blada suggested that once HVAC has corrected

the problem with the valves to submit a new work order to have the ceiling
tiles replaced.
7.
