Element 1

Plan investigation.

Performance criteria

1.1 Prepared work plans include all necessary procedures and resources to investigate failures and meet all industry codes and statutory requirements.

1.2 Plans are in a format agreed by the customer and industry, and define locations and types of plans.

1.3 Plans and specifications are complete, concise, and legible and reflect identified risk with hazards being identified and eliminated, isolated, or minimised.

Element 2

Gather plant and system failure information.

Range System Control and Data Acquisition (SCADA), reports, records, observations, inspections, history records.

Performance criteria

2.1 All relevant information on plant and system failure is identified, collected, and collated.

2.2 Information sources are identified.

Element 3

Analyse results and determine cause.

Range cause, probable cause.

Performance criteria

3.1 Investigation information is analysed.

3.2 Cause of failure is identified.

Best option to remedy failure is identified and recorded in the required format.

5.1 Recorded information is accurate, complete, concise, and legible.

Investigation information is recorded in the required format and filed in the correct location and within scheduled timeframe.

6.1 Recommendations to remedy plant and system failures are made in accordance with plan requirements.

Providers must be accredited by NZQA, or an inter-institutional body with delegated authority for quality assurance, before they can report credits from assessment against unit standards or deliver courses of study leading to that assessment.

Industry Training Organisations must be accredited by NZQA before they can register credits from assessment against unit standards.

Accredited providers and Industry Training Organisations assessing against unit standards must engage with the moderation system that applies to those standards.

AMAP also includes useful information about special requirements for organisations wishing to develop education and training programmes, such as minimum qualifications for tutors and assessors, and special resource requirements.
