CARE COORDINATOR

Position Summary:
The Care Coordinator will be responsible for identifying newly diagnosed, out of care HIV+ inmates and high risk
negative inmates and connecting the inmate to care and community resources upon release from jail, providing
health education and risk reduction counseling, and tracking inmate retention in care after release.
Qualifications:
Bachelor’s degree in social work, psychology, sociology, or other related field. Ability to establish working
relationships with clients and professionals. Knowledge of HIV and/or corrections system. Ability to collect and
analyze data in a complex environment and develop reports that can be shared with the community at large
regarding outcomes of the project. Good verbal and communication skills. Ability to work with other agency
personnel in implementing the project. Good problem solving skills. Must pass background check and gain
clearance by Cuyahoga County Corrections Center
Responsibilities/Requirements:
Good communication and interpersonal skills. Ability to work with a wide range of individuals. Ability to work
independently with minimum supervision. Ability to work compassionately with a diverse population.
Commitment to the mission of Care Alliance Health Center. Work with jail based HIV tester and jail medical staff
to identify HIV positive inmates and high risk negative inmates in Cuyahoga County Corrections Center.
Complete assessments of eligible inmate’s needs for medical care and community resources. Link inmate to
resources before release. Provide individualized health education and risk reduction counseling throughout
incarceration. Upon release, ensure inmate has needed linkages to care and social services. After release, follow
up with inmate to track current usage of medical care and community resources at specified intervals per project.
Form collaborative relationship with community based Ryan White medical and social service agencies. Gather
and analyze data in accordance with specific guidelines of the project and document according to guidelines.
Other duties as assigned by the supervisor.
