You are to work on this assignment individually.
1. Attached is a blank STAT 101 Student Questionnaire. Develop a coding
form for this questionnaire. There will be 120 questionnaires coming in
and each should be identified uniquely. On the blank questionnaire itself
indicate the columns you will use to record a unique 3 digit ID number
and the data for each of the questions. If the response to the question is
a number, be sure to leave enough spaces to accommodate a reasonable
response. If the response to the question is not a number, indicate
what numbers you will use to identify each answer. Also indicate what
numbers you will use to indicate an omitted or unintelligible answer.
Age should be recorded as whole number of years. Height should be
reported to the nearest centimeter.
2. The first three completed questionnaires are attached. On each of the
questionnaires use a colored pencil to mark the form for data entry using
the template you developed above.
3. In the space below, give the first three lines of data for the three completed questionnaires.
