delAware – Summer 2010 – Transition A Transition Guide for Parents “Parents really struggle when their child’s entitlement to formal education is coming to an end,” observed Wendy Claiser, program coordinator of TEEM Employment Services. An evaluation she conducted in 2009 found that many parents of children with disabilities feel unprepared for and uneducated about the process of helping their children transition from school into employment To help parents navigate this process, Wendy created “A Transition Guide for Parents.” The brochure includes information about providers and the roles and rights of families in the transition process; a checklist of steps for parents and students to take; and places to take notes and record information from transition coordinators and case managers.
