Directions:
The Noel-Levitz Student Satisfaction Inventory (SSI) allows each educational institution
to customize a set of questions for inclusion as part of the student survey administration.
The questions below replace the correspondingly numbered blank questions on the
survey form. Responses to the following questions should be recorded directly on the
survey form answer sheet.
Note: Do NOT provide your social security number where it is requested on the back of
the SSI survey form. Responses to the survey questions are intended to be anonymous.
