for the departments that don't currently meet.

health management in select markets throughout the county. This joint venture is knows as
Trinity Health Partners. Establishing Trinity Health Partners will provide Trinity sponsored
ACOs the opportunity to enter into more advanced population health contracts with payer
partners to improve and coordinate care. Trinity will be deploying these new capabilities
through local Joint Ventures in their RHM markets.
NEW - SJRMC Medical Record Completion Policy – You can find this policy attached and at
www.sjrmcmedstaff.com.
In an effort to thoroughly communicate this new policy, we are including this information in
this month's newsletter again.
Mishawaka and Plymouth recently modified policies for Medical Record Completion, which
includes new time frames for completion. One thing to note is the inclusion of coding and
documentation queries as items that require completion (usually a "no change" or a brief
record update is all that is required.) The change in time frames will be implemented now,
but the enforcement of time frames will not transition until June 1 in order to give clinicians
time to adjust to the changes. (Suspension time frames under the previous policies will
continue to exist and be acted on.) Suspension for incomplete records remains a rare
occurrence at the Mishawaka and Plymouth facilities and something that both medical staff
leadership and administration wish to avoid.
Interventional Radiology Consultation Requests
Interventional Radiology (IR) is not staffed 24/7 but have a call schedule if needed after
hours and weekends. IR consults should be called directly to the IR physician on call as
listed in the MDsyncnet on call lists. Non-urgent consults that occur after hours may be
called same as other consults the next morning. Thank you. Constance L. Nichols RN, BSN,
MBA, Administrative Director Surgical & Procedural Services, Phone: 574-335-2496,

Data Loss Prevention and Access Monitoring Safety and Security Programs
We would like to remind the medical staff about two programs to protect our patient and
provide security and safety. The Data Loss Prevention program requests that you be concous
of the content of your emails and texts not to include personal health information (PHI) or
attachments. Please also remember that access to the electronic records is also monitored on
a regular basis. This program is also references as the same last name report. We are only
allowed to access records for which we are providing care and not for looking up friends and
family records.
