Primary Dx: COPD
Secondary Dx: Chronic bronchitis, restless leg syndrome, type 2
diabetes
Medication:
Ventolin (Albuterol) q4h
Medication:
Qvar (beclomethasone
diproprionate HFA) 80mcg (1
puff)
Purpose:
Bronchodilator to open airways
and allow patient to breathe
easier
Side Effects:
-Restlessness
-Nervousness
*Client is also receiving Requip for
RLS and Ativan, Klonpin and
Wellbutrin for nervousness and
anxiety which may be r/t the side
effects of Albuterol
Assessments/Interventions(2)
-Check heart rate – Albuterol can
cause tachycardia
-Assess mental status – Albuterol
can cause confusion and agitation
-Lung sounds – have a baseline so
that improvement can be noted
Labs to Monitor
-Blood sugar: Albuterol
can cause
hyperglycemia and in
rare cases diabetes.
Medication:
Klonopin (clonazepam)
0.5 mg (1 tab) PO every day
Demographics: 49 yo female
Medication:
Requip (ropinirole HCl) 0.25 mg
(1tab) qid
Purpose:
Corticosteroid used to decrease
inflammation in airways which
decreases wheezes
Side Effects:
-Yeast infections in mouth
-Mental/mood changes
**May be reason for Nystatin
and for antianxiety and
antidepressants
Assessments/Interventions(2)
-Lung sounds – any worsening
lung sounds should be noted
-Check mouth for signs of sores,
white patches and dryness.
Labs to Monitor
-Blood sugar:
corticosteroids can
increase insulin
resistance
Purpose: Benzodiazepine used to
treat panic and anxiety
**Panic/anxiety may be caused
by Albuterol use
Purpose:
Domapine agaonist used for
treatment of restless leg
syndrome
Side Effects:
-Anxiety, nervousness
-Pain in arms or legs
**Pt receiving Morphine, Phenergan
and Dolophine PRN pain which may
be r/t Requip
Assessments/Interventions(2)
-Assess mood and mental status –
make sure that pt isn’t nervous,
anxious before administering med
because it could make anxiety worse
-Pain assessment – may need to
administer pain meds before giving
Labs to Monitor
-Liver function: should
be titrated with
caution in those with
altered liver function
Side Effects:
Constipation
-Drowsiness
*Pt also taking Miralax
and Colace to
counteract
constipation
Allergies: acetaminophen,
codeine, meperidine,
penicillins,
prochloperizine
Medication:
Nycostatin Suspension PO QID
Purpose: Antifungal used to treat
thresh in the mouth that often
occurs from use of inhalers
Side Effects:
-Diarrhea
-Nausea/Vomiting
*Pt also receiving Zofran PRN
nausea r/t Nystatin
Assessments/Interventions(2)
Check hydration status – N/V/D
very common with Nystatin
Check fungal growths in mouth to
create baseline
Labs to Monitor
-Monitor electrolytes if patient is
experiencing diarrhea/vomiting
to make sure the patient is not
becoming dehydrated

Make sure respirations are above 12
before administering
Assess eye – pts c narrow angle
glaucoma should not use Klonopin
