Must have a current IEP 2. Must be 16 years or older to be eligible for an Individual Placement 3. Must be 14-15 years old to be eligible for a Special Project Placements: Individual/summer (white folder): Individual Placements will open beginning November 3, 2014. Students who are 16 years of age and older are eligible to earn up to 100 hours (earning $9.00/hr.) working at a business located within their community. Student placement sites are determined based on the student’s interests and skills, as well as availability and signed agreement with local businesses. All students seeking an Individual Placement need to complete and submit the following documents and meet the below criteria: 1. Copy of Social Security Card (a birth certificate may be accepted in place of a SS card) * (if student was placed during the 2013-2014 school year they will not be required to submit a copy of their SS card). 2. Copy of current picture identification (ex. student ID, school record, CA ID, driver’s license, passport, or military ID will be accepted). 3. Copy of current IEP Information/Eligibility Form 4. WorkAbility Student Application 5. Current Resume (on resume paper) 6. 3 applications from businesses (applications do not need to be filled out. If business directs student to their website for an application and the application is not able to be printed, student needs to notate “online application and business name” on their white folder) 7. Job Readiness/Scan Self Evaluation 8. Verification of 90% school attendance 9. Work Permit Application (student may pick this form up in their school attendance or counseling office) parent/guardian signature required 10. Student Profile Form Student Job Readiness/Scans Evaluation Student Pre-Employment Competency Checklist Make sure student folder is complete before contacting assigned Job Developer to pick up Special Projects (red folder): Special Projects provide support and extra help to Butte County communities. For example: YMCA events, Durham Spookfest, Gold Nugget Days, Clean-up Projects, etc. give students an opportunity to gain work experience. Special Projects may begin in September 2014 and will continue throughout the school year. The students will earn $9.00/hr. The Job Developer assigned to your school is in charge of selecting the Special Projects for the year and giving teachers and students ample time to complete all necessary documents. Special projects are designed specifically for 14-15 year old high school students (16 and older students who are not ready for an individual placement this year, may apply for a special project, but those students who participate in a Special Project will not be eligible for an individual placement). Special Projects will not exceed 10 hours of paid work experience. All students seeking a Special Project need to complete and submit the following documents: 1. Completed Student Placement Contract (parent/guardian signature required) *Teachers please make sure all information is complete for each student before the assigned Job Developer picks up the student folders. **Job Developers are not responsible to fill out the above paperwork for students participating in an individual or special project placement. If the student is unable to write, the parent will be required to sign a release form, which will be provided, giving permission for either the teacher or job developer to fill out the students required paperwork. Placement Deadlines: Individual Summer Placements for seniors: March 27, 2015 Individual Summer Placements for non-seniors: April 17, 2015 Special Projects: Deadlines are posted on all Special Project flyers ***Students who fail to complete folders by the given deadlines are not guaranteed a placement for the summer. ****Students may only be placed once for the 2014-2015 school year. Students may not participate in more than one placement this year. All 16 and older students are encouraged to participate in an individual placement and all 14-15 year olds are encouraged to participate in a special project. *****In the case of student referrals exceeding WorkAbility funds for placement, students will be placed in the following priority: graduates, non-graduating seniors who have not had prior placements, juniors/sophomores who have not had prior placements, date completed folder received. Materials and Supplies: All material and/or supply requests need to be emailed to Nicole Beall for approval (nbeall@bcoe.org). Please provide a detailed summary of the purpose for each item being requested. Please be mindful that the WorkAbility Program can provide only career and vocational based materials for high school students with current IEP’s and should be used to supplement, not supplant. Resume Paper will be provided to each SPED teacher for student use. WorkAbility will strive to continue to provide proper work attire based only on dress requirements for their job placement and students who are in dire need. WorkAbility is not funded to provide work attire for every student. WorkAbility can provide bus passes for students to get “to and from” their work site as well as teacher career related field trips, with the intent of mobility training (a detailed summary must be provided and emailed to Nicole Beall prior to purchase). WorkAbility Services Offered for 2014-2015: Each school will be assigned a job developer and your assigned job developer will be your primary WorkAbility contact (please see last page for your assigned Job Developer). Classroom Presentations: Presentations will be scheduled beginning September 2014 through October 2014. 9th-12th graders will receive an introduction to WorkAbility presentation paired with a presentation relating to required soft skills, social skills, and pre-employment skills to assist in preparing students for competitive employment. Future presentations may include filling out practice applications (paper/online), employer based forms and completion of I-9 and W-4 Forms, along with the understanding of those forms. Guest Speakers: WorkAbility will strive to provide quarterly guest speakers this year upon request by the teacher and availability of requested guest. Please email your assigned Job Developer to assist you with setting up a guest speaker of your choice. Personal Data Wizard: Each student is able to have an account with the Personal Data Wizard online. This program allows students to create a resume, cover letter, thank you letter, and an annual budget. It also provides study tests for the California Drivers Test, Armed Services Vocational Assessment Battery, Interest Assessment, and a sample Employment Test. In addition, Personal Data Wizard offers several documents to download that are useful for students transitioning into independence and adult life. The downloadable documents are listed below: (1) Now that you are 18: This file contains information in a lesson outline regarding the changes that occur when a person moves from being a minor to becoming an adult. It talks about the different responsibilities and consequences that pertain to an adult 18 years of age. There are a few self-evaluating quizzes within the material. (2) High School Recommendation: This file contains a form to be given to the student's teachers for their evaluation of his or her performance in class. The forms can be used as letters of recommendation along with the student’s resume. Students should keep the good ones and throw out the bad ones while learning what they can do to improve in that class. (3) Selective Service Information: This is information regarding the selective service and a male’s responsibility to register at age 18 or before. The student can register online by clicking on the link to the Selective Service. It takes about 5 minutes. (4) Buying a Used Car: This document explains some of the terminology of automobiles. It poses a number of questions a used-car buyer should ask themselves and the seller. The lesson has a self-evaluating test. (5) Banking and Credit: This document explains how to establish credit, what happens if you lose credit, and defines some basic banking terminology. It also has a selfevaluating quiz. (6) Blank Job Application: This is used to provide the user with a template of an application for employment. Once completely and accurately filled out and printed, the application can be used when filling out other job applications. The typed application should not to be used as an actual application for employment. The application can be saved on disc or hard drive for editing and updating later. (7) Reference Request: This is a form letter to be filled out online to request permission to use a particular person as a reference. (8) Letter of Recommendation Request: This is a form letter to be filled out online to request a letter of recommendation. (9) Questions and Answers about SSI: This is a series of frequently asked questions regarding SSI and cross-referenced with the appropriate SSI publication. (10) Preparing for the Interview: This document has lessons in outline form on preparing for a job interview. It contains helpful hints and a list of frequently asked interview questions with an appropriate response. Once the account has been created, the student’s login information will be emailed to you. Verification of Reduced Fee CA Identification Card: WorkAbility would like to encourage all students to apply for a California ID Card as students now must have a valid CA ID in order to cash their checks or open a bank account. WorkAbility has a form for students who are pursing to obtain a CA ID card, which brings the fee down to $8.00 (the parent and/or student is responsible for paying the $8.00 DMV fee, not WorkAbility). Please email Nicole Beall (nbeall@bcoe.org) to request the proper form. The email must include the student’s full legal name, date of birth, and current address in order for the form to be completed and submitted to the student. (Instructions for parents and students are notated on the reduced fee form). Job and Transition Fair: WorkAbility will strive to continue to hold a Job and Transition Fair each year for all high school students within Butte County. The fair was held in Chico last year and this year we hope to hold the fair in Oroville. COPS II Assessments: This inventory will help your students find out what kind of jobs best fit their interests. The results will help your student in choosing activities, training, and school courses which will help prepare your students for the jobs they are interested in. IDEAS Assessements: With the IDEAS workbook, students will learn about their interests and different types of careers and jobs they might enjoy. 12 Grade Transition Handbooks: WorkAbility will be offering a Transition Handbook online for graduating seniors. This handbook will include the following resources: Local Resource Information (ex. Dept. of Rehabilitation, Far Northern, Alliance for Workforce Development, etc.), Local College Information, Personal Record Keeping Reminders, Employment Tips, Online Career Information, Budget Worksheet, and more!
