GENERAL INSTRUCTIONS
The accurate and fair completion of this form should be considered one of the most important individual
assignments you are responsible for during the course of the project. Students are expected to provide,
meaningful, fair, and comprehensive evaluation of the contributions of all team members. How well you
complete these evaluations will be reflected in your individual grade.
