HUBENAK TARDY POLICY

7:45 – Tardy Bell
• Students who are not in the building by 7:45 will be considered tardy.
• Morning math practice begins at 7:00am. Students who are tardy miss out on the morning math practice.
This is a crucial piece of math review and STAAR test preparation.
7:55 – Instruction begins
Instruction begins promptly at 7:55. Students who are not seated and ready in class will miss out on new
instruction. Tardy students also cause disruptions to the teaching and learning that has began.
Consequences for Tardies
Tardies will start over each grading period.
1st Tardy – warning
2nd Tardy – silent lunch
3rd Tardy – silent lunch and no recess
4th Tardy – after school detention
Additional Tardies – after school detention, teacher contacts Assistant Principal, Assistant Principal contacts parents
Tardies affect the Perfect Attendance Awards.
ATTENDANCE POLICY
• Students should attend school on time every day.
• If your child does not arrive to school by 9:30am, they will be considered absent.
• In the event of an absence, please inform your child’s teachers. When the child returns to school, please
send a note to the school explaining the absence.
• If your child is absent or removed from school early, the parent will need to make arrangements with the
teacher for the student to make up their missed work.
• Makeup work may only be completed when the child returns to school. All graded work must be
completed independently at school.
• Please schedule all medical appointments after school to avoid your child falling behind.
• Accumulated times of early pick-up is calculated by minutes in loss of instructional time.

VISITORS & TRANSPORTATION CHANGES
Visitors
• All visitors must enter through the front doors.
• You will sign in, have your ID scanned, and receive a visitor’s badge.
• Visitors must only go to the designated areas they have signed in for.
• Visitors may not go to their child’s classroom without a scheduled conference unless there is a grade level
event occurring.
• Visitors are not allowed on campus testing days. (Letters will be sent home.)
Transportation Changes
• Transportation changes must be made by 2:00 (or by 11:00 on early dismissal days.)
• Parents must send a written note or call the front office. Please do not send an e-mail because your child’s
teacher may not receive it in time.
• Parents may not walk up and take your child out of the car rider line, You will be required to come into the
office to sign them out.
• Parents may not take their child out of the bus line.
• Only people listed as Emergency Contacts will be able to sign out your child.
WALKERS & CAR RIDERS
Walkers
• Walkers will be released at the designated area to walk home even if there is not a parent present.
• Walkers will still walk in bad weather if a transportation request has not been made by a parent.
• Once walkers cross the street, Hubenak staff are no longer responsible for them and they are on
their own if a parent is not present.
Car Riders
• Parents will be required to have the bright green car tag in the pick up line. If a parent (or
someone picking up the child) does not have it, they will have to park and come inside the
building before taking their child.

HUBENAK DRESS CODE POLICY
All students are required to wear uniform dress. The uniform dress consists of:
Shirts
• School or grade level spirt T-shirt
• Solid & collared: White, navy blue, light blue, black, or red
• Long sleeve undershirts must also be solid white, navy blue, light blue, black, or red.
Sweaters/Jackets/Hoodies
• Solid

• Hubenak spirit wear items are allowed
• Hats and hoods must be removed while inside the building.
• Non-dresscode items may only be worn while outside.
Logos MUST be smaller than a quarter!
HUBENAK DRESS CODE POLICY
Bottoms
- Denim pants, capris, shorts, skirts/skorts
- Khaki, navy blue, or black pants, capris, shorts, skirts/skorts, dresses
- Jeggings/leggings are not allowed unless worn under a skirt.
- Athletic/yoga/sweat pant materials and styles are not allowed.
- Shorts, skirts/skorts, and dresses MUST be fingertip length or longer.
If a student arrives to school in clothing that is not in dress code, they will receive a
note in their agenda.
If a student arrives to school in clothing that is considered to be inappropriate or a
distraction, they will be asked to change in the nurse’s office or call home for an
appropriate change of clothes.
HUBENAK DRESS CODE POLICY
Shoes
- Tennis shoes must be worn on PE days.
- If a student does not wear tennis shoes on PE days, they will receive an alternative activity in PE class
and a note home from the PE teacher.
- Shoes must be safe for recess.
- Flip flops and backless shoes are not allowed.
Additional Information
- Rolling backpacks are not allowed.
- Earrings on male students are not allowed.
- Girls should not wear long, dangling earrings that cause a safety concern.
- Extreme haircuts (such as mohawks, psychedilc colors, lines/letters/designs shaved in the hair) are
considered inapproriate. A note will be sent home requesting the hair to be fixed within 2 school days.
SPECIALS INFORMATION
This year, your child will go to art, music or PE on a weekly rotation. The schedule will be FIXED on Monday – Thursday to
help your child remember which days to have the correct shoes for PE.
Fridays will rotate. However, if we have a Monday holiday, the instruction will always be made up the following Friday.

POLICIES
Content Grades in PE, Music and Art
Each student will receive two grades in art, music and physical education: academic performance and conduct.
Performance - Mastery of the instructional objectives will be the determining factor in assigning performance grades.
Three grades will be taken during each 9-week period. At any point during the grading period that the student’s grade
falls below an S, he/she will receive a progress report, or documentation on a specials conduct note, that should be signed
by the parent and returned promptly. Please see page 50 of the LCISD student handbook for specific guidelines regarding
specials assessment.
Conduct - Behavior will be used to determine the conduct grade. A student’s specials conduct grade is separate from the
classroom conduct grade. Specials teachers will communicate conduct marks with a written note that will be stapled in the
student’s agenda. Each note should be signed and returned promptly. Conduct grades are aligned with the grade level
policies. They are as follows:

*Please note that specials grades are unique in that for both conduct and performance grades, students must participate

PE PROCEDURES
MEDICAL EXCUSES - IF A STUDENT NEEDS TO BE EXCUSED FROM PARTICIPATION
IN PHYSICAL EDUCATION ACTIVITIES FOR MEDICAL REASONS, A WRITTEN EXCUSE
FROM THE PARENT MUST BE PROVIDED IF THE STUDENT IS TO BE EXCUSED FOR 2
OR LESS DAYS (ONLY ONE PE DAY). IF A STUDENT IS TO BE EXCUSED FROM
PHYSICAL EDUCATION ACTIVITIES FOR MORE THAN 2 CONSECUTIVE SCHOOL DAYS
(MORE THAN ONE PE DAY), PLEASE PROVIDE A WRITTEN EXCUSE FROM THE
DOCTOR.
APPROPRIATE SHOES – ON DAYS WHEN YOUR CHILD HAS PE, IT IS EXTREMELY
IMPORTANT THAT YOUR CHILD WEARS FLAT, RUBBER-SOLED, TENNIS SHOES THAT
ARE FIXED FIRMLY ON THEIR FEET, COVER THE ENTIRE FOOT, AND DO NOT LEAVE
SCUFF MARKS ON THE FLOOR. YOUR CHILD WILL BE RUNNING, JUMPING, AND
DOING VARIOUS OTHER PHYSICAL ACTIVITIES IN THEIR TENNIS SHOES. NOT ONLY
IS THIS A HUGE SAFETY PRECAUTION AND A NECESSITY TO MAINTAIN THE GYM
FLOOR, BUT IT IS ALSO PART OF YOUR STUDENT’S PE PERFORMANCE GRADE.
EACH TIME YOUR CHILD DOES NOT WEAR THE PROPER SHOES TO PE, HE/SHE WILL
RECEIVE A MARK IN THE GRADE BOOK. THREE MARKS IN A SINGLE 9-WEEKS WILL
CONSTITUTE A DROP IN HIS/HER PE PERFORMANCE GRADE.

• You do NOT have to attend meetings! Be involved as little or as much as you want.

• If you join, you will receive early admittance into school events (such as the Kindergarten celebration).
Students Incentives
• If 60% or more of your child’s class has at least 1 family member join, that class will earn a homework
pass.
• If 100% of your child’s class has at least 1 family member join, that class will earn a popcorn part and a
read aloud by an administrator.
(1 family member can only be counted for 1 students.)
BIRTHDAY INFORMATION
Birthday Treats
• The state law recommends that birthday treats be served after the lunch period so that it does not spoil
students’ appetites for a nutritious meal. In 4th grade they will be eaten at the end of the lunch period
after the students have eaten.
• Birthday treats must be store bought.
• The treat must be able to be served, eaten, and cleaned up in 5 minutes. Please do not bring any items
that require utensils (such as cakes.)
• Only 1 treat per student should be brought, and there must be enough for every student in the class to
receive one. Please send class treats to school with your child.
• Delivery to students of flowers, balloons, treat bags, and other such items is not allowed at school.
Birthday Invitations
• Birthday party invitations are NOT allowed at school.
• Your child’s homeroom teacher will have a birthday list. If you want your child to be able to receive
birthday initiations, please fill out the information to be put on the birthday list. The teacher will send
out the birthday list to those who have signed up so they can send out invitations.
SCHOOL MEALS
LCISD Charge Policy:
Students may charge meals up to the value of 5 breakfasts and
5 lunches or $15.00. A low balance reminder will be sent
home to elementary students and an automated phone call
will occur, for all students, when a negative balance appears
on the student’s account. If a student reaches the charge limit
of $15.00, an alternate breakfast of a cereal and milk will be
provided at a cost of $0.50. During lunch service a cheese
sandwich and milk will be provided at a cost of $0.50 until the
negative balance is paid. A la carte items are not available for
charging. This policy will be consistent for all grade levels on
all campuses.
Payment Options:
Parents are encouraged to put funds in the student account prior to meal service. Payments can be made at each campus
cafeteria with cash or a check. Payments can also be made online. Parents can set up balance reminders to ensure there

If your child is going to eat breakfast at school, please make sure they arrive to school early. The later they
arrive, the longer they will wait in line. This will result in lost time completing their morning math sheet.
COMMUNICATION
Agenda – Parents should check and sign their child’s agenda every day. The agenda may have notes
from the teacher in addition to daily behavior notes. If the agenda is not signed, the student will walk 1
lap at recess for this missing signature.
Email – Please allow 24 hours for a response.
Newsletter – This will be e-mailed to you every week by your child’s homeroom teacher.
Conference – Our conference period is 12:50-1:40. Please contact your child’s teacher to schedule a
conference.
Husky Folder – Graded papers and flyers from the school will be sent home every Thursday. Please
review all graded work with your child and return the folder on Friday.
PARENT CONFERENCES
1st 9 week Conferences
• November 5 and 6 are early release days and reserved for parent conferences.
• Your child’s homeroom teacher will contact you as it gets closer to sign up for a time.
• These conferences are mandatory and a district policy.
• You must attend a parent conference in order to receive your child’s first report card.
Additional Conferences
• If you need additional conferences throughout the year, you may contact your child’s teacher(s) for
scheduling.
• Teachers may also schedule additional conferences with parents throughout the year if needed.
SCHOOL SUPPLIES
Notes about the school supplies list:
- The following items will be collected as classroom/community supplies:
Paper, tissues, hand sanitizer, and ziplock bags
- All other supplies will be kept by your child. It is their responsibility to take care of
their supplies.
- Additional supplies not currently being used will be stored and given to the students
as they need it or returned at the end of the school year.
- Your child will also need large erasers, especially for math. (This is not on the school
supply list.)
- Your child will most likely need additional glue stick and glue bottles.
- Swim suit – The 4th graders will attend the swim program from May 11-31. They will
not need the swimsuit until then. If you would like to purchase it now, girls will need
a 1-piece and boys will need swim trunks. They are NOT allowed to wear coverups in
the pool. Information will be sent home in April with more details.
SNACK INFORMATION
When is snack time?
• After specials @ 1:45 pm.
• Students must unpack their snack in the morning. They will not be allowed to go to their backpack to get it.
What can they eat?
• Nutritious snacks
• Examples: Fruit (grapes, apples, banana), fruit snacks, bite sized crackers, pretzels, granola bar
• NON-Examples: Chips, cookies, cupcake, chocolate
• Foods they are able to eat with their hands.
• NON-Examples: pudding, applesauce, fruit cup
• Foods that do not need to be refrigerated – They will be in their tub all day.
• NON-Examples: gogurt, cheesestick
** Students should be able to eat it in 5 minutes or less.
** Students must also be able to continue working while having their snack.

What can they drink?
• Students may have a bottle of water that they can drink from throughout the day. They may refill it during restroom breaks
and lunch.
Students who do not follow these guidelines will be asked to put their snack away.
PEANUT FREE CLASSROOM
Parents, please remember that all snacks that are consumed in the
classroom must be peanut/tree nut free. Students can bring
peanut products to eat in the cafeteria at lunch but NOT in the
classroom.
GRADING POLICY
Students will be graded in the following subjects:
• Math
• Science
• Social Studies
• Reading
• English Language Arts (Writing & Language Arts)
Each grading period:
• Minimum 8 daily grades/quizzes in each subject
• Minimum 2 major grades in each subject
Grade Weight:
• Daily grades = 70% of student’s average in each subject
• Major grades = 30% of student’s average in each subject
RETEACH/REASSESS POLICY
• A teacher shall provide corrective instruction and ONE reasonable opportunity to reassess
failure to master the TEKS/Curriculum on each daily grade.
• A teacher shall provide corrective instruction and ONE reasonable opportunity to reassess
failure to master the TEKS/Curriculum on each test grade. Teachers will re-teach and allow
students an opportunity to retest the objectives that were not mastered.
• The highest possible grade that can be earned and recorded on the reassessment is a 70. The
teacher will make a note in the electronic grade book of the dates and grades of the
reassessment.
• EXCEPTIONS: The following assessments are not subject to the reassessment guidelines: DRA,
TPRI, Running records, student projects, Elementary common assessments.
HOMEWORK POLICY
Students will receive homework in the following subjects:
Math – every night Monday-Thursday
• Fact fluency every night (Fluency quiz on Fridays)
• Math page every night
 2 nights = spiral review page (previously learned material)
 2 nights = focus page (material learned that week)
Reading – every night Monday-Thursday
• Will alter each week between completing their reading log and completing reading passages.
Writing – every night Monday-Thursday
• Will alter each week between spelling, revising/editing practice, and writing practice.
The students will write down the specific homework assignments for each subject into their agenda.
If a student does not complete their homework, they will receive a color change for it and will be expected to
complete it at recess. This will also affect their conduct grade on their progress report and report card.

4 GRADE CONDUCT PROCEDURES
Warning = The student will place their agenda in the Stop Box as a physical reminder to fix the misbehavior.
1st conduct mark = blue = walk 2 total laps at recess
2nd conduct marks = yellow = walk 4 total laps at recess
3rd conduct marks = red = walk 6 total laps at recess, Parent Behavior Note will be sent home
(The students will walk 2 laps for every conduct mark they receive.)
The teacher will record each conduct mark in their agenda. The conduct codes are located below and on a sticker in the
front cover of your child’s agenda. These codes are also used for assigning conduct grades.

HUSKY HANGOUT
Q: What is it?
A: 30 minutes to hangout with friends and play on electronic devices brought from home.
Q: When is it?
A: It will be held the Friday after each progress report and report card is sent home.
Q: Who can attend?
A: Every student that has all S’s on their progress report or report card.
SUCCESS ASSEMBLY
Q: What is it?
A: An awards ceremony to recognize perfect attendance, A and A/B Honor Roll, and students who have earned
citizenship awards. (More than 1 tardy will disqualify a student from receiving perfect attendance.)
Q: When is it?
A: It will be held at the end of every grading period. An announcement will be sent home.
Q: Who can attend?
A: All students will attend. Parents and family members are welcome to attend as well.
4TH GRADE INCENTIVES/REWARDS
- Gold
Gold means they have gone above and beyond what is expected of them. Please
encourage your child to strive for gold and reward them for doing so.
- Dog Bones
The class will receive dog bones for excellent behavior in and around the school. When
the class earns 10, they can be redeemed for a prize from Mrs. Boarman.
- Husky Bucks
The students will earn husky bucks for exhibiting positive behaviors. They can be
earned from any staff member.
4TH GRADE INCENTIVES/REWARDS
Husky Store
Students can purchase items with their husky bucks or trade is their husky bucks for
larger denominations of money on Monday mornings.
$20 = Treasure box (small items and passes)
$50 = Homework pass
$100 = Picnic Lunch with a friend (This will be scheduled and supervised. The
students will be able to invite 1 friend to have lunch with them at the picnic
tables outside.)
Grade Level Auction
We will hold a grade level auction at the end of each semester.

WRITING STAAR TEST
Information:
• Day 1 – March 29: 1 Written Compositions & Revising & Editing Multiple Choice
(4 hours total for both parts)
• Day 2 – March 30: 1 Written Compositions & Revising & Editing Multiple Choice
(4 hours total for both parts)
How to help your child prepare all year:
• Make sure that your child completes all writing homework correctly.
• Review writing assignments that are sent home. This can be a composition that they wrote or a
revising/editing passage.
• Remind them that they can take their writing notebook to and from school to help them with their
writing.
• Encourage them to go on istation and access the writing portion. This helps students learn to use the
writing process more efficiently and motivates them to write more!(Login information will be sent home.)

4 GRADE MATH STAAR TEST
Information:
• May 9
• 4 hours for completion
How to help your child prepare all year:
• Make sure they are completing their homework correctly every night.
• Remind them to bring their math interactive notebook to and from school as a tool to use on their
homework.
• Make sure they are getting to school early in order to complete they morning math sheet.
• Review graded work with them (even if it’s a passing grade!) This could help turn their B’s into A’s!
• Review their morning math sheets with them. These questions are similar to what they will see on the
STAAR test.
• Make sure they memorize and practice their multiplication facts! (If they do not have them memorized,
start practicing NOW. We use multiplication in almost all math concepts in 4th grade!)
**Our multiplication unit begins the 1st grading period. If they do not have their facts memorized,
solving larger multiplication problems such as 76x89 will be challenging!

4 GRADE READING STAAR TEST
Information:
• May 10
• 4 hours for completion
How to help your child prepare all year:
• Make sure that your child is reading at least 20 minutes each night.
• When your child is given a reading passage for homework, have them show their thinking to
prove their answer choice.
• Review reading assignments and/or assessments that are sent home.
• Encourage them to utilize Raz-kids and istation at home. Both of these resources are great tools
that your child can use to practice their reading skills. (Login information will be sent home.)

SUCCESSFUL IN 4TH GRADE:

Check their agenda daily and enforce classroom expectations at home.
Review graded work that comes home in their Husky Folder. (For example – if
they receive a 78 on an assignment, we most likely did not go over it in class.
