2015-2016 SUPPLY STAFF RENEWAL PACKAGE
Dear Supply Staff Employee:
We hope you have enjoyed working in our schools this year. Preparations are underway for the 2015-2016 school
year. Please review the information below as our information package has been REVISED. Completion of the 20152016 Supply Staff Renewal Package is required by Monday, June 29, 2015.
PROFILE
All supply staff employees (Occasional Teachers, Supply Designated Early Childhood Educators, Supply
Educational Assistants, Supply Secretaries) are required to maintain an updated Smart Find Express (SFE) profile,
i.e. callback numbers, board email, etc. Your status will remain active in SFE until notified otherwise. Updated
information on SFE is necessary to confirm your intentions to continue working as a Supply Staff member with our
board for the 2015/2016 school year. If you are NOT returning for the 2015/2016 school year, please email Sonja
Ruby at Sonja.ruby@dcdsb.ca and/or Isabel Farrugia at Isabel.farrugia@dcdsb.ca with your intentions and your
name will be removed from the appropriate Supply list and SFE.
Employee Information Change Form
All employees requesting a name or address change are required to submit a completed Employee Information
Change Form (Form #4301). This form can be found at: https://mydcdsb.ca/cec/policies/SitePages/Forms.aspx.
Click on Form Name to list numerically. Scroll to the bottom of the list to advance ►to the next page. Form 4301 is
found on page 3 (61-90). For name changes, a copy of a Confirmation of Social Insurance Number (SIN) letter, a
copy of a Notice of Assessment or a copy of Canada Revenue documentation with your SIN assigned to your new
legal name, must accompany the Form 4301.
Annual Offence Declaration –NEW PROCESS (eServe)
As per Numbered Memo HRAS1415-083, our Annual Offence Declaration process has been revised.
In accordance with current legislation, all employees of the Durham Catholic District School Board are required to
complete an annual Offence Declaration each year. Continued employment with the Board is contingent upon
submission of your completed Offence Declaration on or before June 30, 2015.
Process for Completion of your Offence Declaration:
1) Watch the e-Serve annual Offence Declaration presentation by clicking on the following link:
https://mydcdsb.ca/staff/Staff%20Library/Annual%20Offence%20Declaration%20e-Serve%20presentation.pptx.
2) Copy and paste the web address/URL into your web browser: https://sdsweb.dcdsb.ca
3) Log on to the e-Serve module using the same username and password that you use to log onto your computer
each day. Enter the required information, click on “electronic signature”, enter “password” and click “submit”.
4) You must select email or print to receive confirmation of completion.
Policy and Procedure Annual Review – NEW PROCESS!
All employees are required to review policies/procedures before returning to work for the 2015/16 school year.
To access them, please follow these steps:



Click on

Go to Applications (top bar).
Select Policy and Procedures Sign Off (from pull-down menu).
Review all policies and procedures listed (right-hand side).
Click on




Complete (√) the form.
Please enter Lina Goldsmith as the Supervisor.
Click “submit”
Process complete.



Safe Schools Initiative
As an educational organization, student safety is priority. In accordance with the Ministry of Education’s
ID Badges expire on June 30, 2015 and a new badge is required for the 2015/2016 school year. Once
you have completed the Offence Declaration, Policy and Procedure Review Sign Off and updated your
SFE profile, you may pick up your 2015/2016 ID Badge.

Pre-printed ID Badges are available for pick in the North and South Boardrooms at the CEC
(650 Rossland Rd. W., Oshawa) on the following dates only:

IMPORTANT INFORMATION

ID Badges will NOT be provided to any employee without confirmation of completion of the
Offence Declaration, Policy/Procedure Sign Off and the return of their 2014/2015 ID Badge. Also,
one additional Photo ID, i.e. Driver’s License, Health Card, etc. must be presented.

All supply staff must present their current school year ID badge upon arrival at each and
every school. Supply staff without a current ID Badge will not be permitted to work in any
school. You will be deactivated on SFE until the renewal process is complete.

If your employment status should change throughout the year or if you are not returning for the
2015/2016 school year, your current ID Badge must be returned immediately.

We encourage you to check your board email regularly to ensure you are receiving information
regarding employment opportunities, pay statements, announcements, etc.
