 Complete Lifestyle Wellness Questionnaire
 pp. 25 & 26
 Make photocopy – so you can sell back the text
 Identify 5 behaviors that could use changing
 Choose 1 as a priority behavior
 Closely monitor that behavior for 4 days
 In writing – journal, excel spreadsheet, whatever
works……BUT IN WRITING
 Write a summary of what you found out about
that behavior
