Ashley Ruocco – Ruocco welcomed all to the call and introduced those at HQ in Germantown. Please continue
to email your attendance to Gail Thomason gthomason@pec1.net. Gail Thomason will also be assisting with the
minutes to the OEC calls; this support is very much appreciated.
The OE Committee Meeting will be held in conjunction with EFCOG ISM&QA Spring Meeting at
Argonne National Laboratory, April 30-May 2. OE Committee Meeting will be on May 1 or May 2
depending upon room availability. A list of hotels near Argonne is included at the end of these OEC Minutes.

SITE OPERATING EXPERIENCE DISCUSSIONS
1. Brookhaven Site Office / Brookhaven National Lab (BNL)

Mike Hauptmann reported a rash of minor automobile incidents where drivers backed into other cars or
stationary objects. His questions Is this occurring at other sites? and Is there an increase in this type of
event or awareness of it vs. 5 years ago? elicited a thought-provoking discussion. BNL’s analysis
resulted not only in safety reminders such as “Look before you back up” but attempts to increase
automobile drivers’ awareness of how limited the visibility can be from the cabs of trucks, forklifts, etc.
The general opinion was that there are more incidents where drivers have not been focused, as well as
increased awareness of distracted driving. One site noted that their Safety Fest included giving
employees the opportunity to climb up into the cabs of heavy equipment so they could experience the
challenges heavy equipment and truck drivers face in seeing the smaller cars around them.
2. Idaho Operations Office (ID) / Idaho National Lab (INL)

Brian Anderson (ID) invited ITG to share information from the recent North PAPR filter cartridge
failure problems. The site uses hundreds of these filters per day.
o ORPS Report: EM-ID--ITG-AMWTF-2013-0003. Occurrence Description: On six different
occasions Powered Air Purifying Respirator users experienced filter breakthrough of the
charcoal media. The breakthrough resulted in carbon dust in the user's breathing zone and sootlike deposits on the inside surfaces of the facemask. The breakthroughs occurred in as little as 15
minutes of use in some cases. The HEPA media remained intact.
o The filter cartridges in use in all cases were PAPR 4001HE Cart Filter, PAPR, OV/HEPA type
filters classified as Combo HEPA/Organic Filter Cartridges and were from two different lots.
ITG is in contact with the manufacture to resolve the issue. A meeting is scheduled for 2/12/13.

o Real-time monitoring was in use and showed that there were no breakthroughs or worker
exposures.
o Manufacturer: North Safety Products manufactured by Honeywell
Lot numbers: W498933 and SFC028141
Mark Wescoat spoke about the successful exercise at the AMWTP Safety Fest where employees had the
opportunity to climb up into the cabs of heavy equipment to experience first-hand the challenges heavy
equipment and truck drivers face in seeing the smaller cars around them. LL: give equipment and trucks
a wide berth when driving near them.
3. Lawrence Berkley National Lab (LBNL)

As part of the LBNL Operating Experience Process Improvement Project, Donna Spencer has created a
list of benchmarking questions to help them in their efforts to improve the lessons learned process.
Ruocco emailed the OEC the Benchmarking Document and posted it on the OE Wiki. Committee
member input is requested and appreciated on LBNL’s Benchmarking Questions.
4. Livermore Site Office (LSO)/ LLNL


Jeff Horning provided information regarding a recall of Fluke 373, 374, 375, and 376 Digital Clamp
Meters that were manufactured between September 1, 2010 and October 1, 2013. The printed circuit
assembly may not be properly fastened to the test lead input jack. This may result in inaccurate voltage
readings, including a low or no voltage reading on a circuit energized with a hazardous voltage that
presents a shock, electrocution or thermal burn hazard. The Recall Notice is located on the OE Wiki
under February’s OEC Call and was emailed with the minutes of this meeting.
Los Alamos Site Office (LASO) / LANL
Bob Stuewe reported Lessons Learned will be submitted regarding Lujan Center Events that will discuss
contamination issues similar to those highlighted in last year’s contamination event at LANSCE where
sample containers and/or parts were reused. Labeling did not make the distinction between technetium
and lutetium-technetium which led to contamination and confusion. Lessons Learned was the need to
reinstate the practice of using “what if?” scenarios.
Nevada Site Office (NSO) / NST / NSTec
Sara Langlois reported that last year, after EM noticed workers becoming too comfortable around heavy
equipment such as forklifts and cranes, the site sponsored a Safety Fair. Workers who did not drive or
regularly work around heavy equipment had the opportunity to climb up into various types of equipment
and see firsthand how difficult it was for the operator to see who and what was around them. The
exercise was well-received.
Sara Langlois also brought to everyone’s attention the importance of guarding Official Use Only (OUO)
and Personal Identifiable Information (PII) such as home telephone numbers and addresses.
Richland (RL) / Hanford Site
David Langstaff reported on ongoing efforts with the Project Management Lessons Learned system,
including Lessons Learned from ARRA-funded work. EM is coordinating the Project Management
Lessons Learned.

Stanford Site Office (SSO)
After hearing about the successes that other sites have had in familiarizing their workers with the
limitations on heavy equipment operator visibility, Mark McDaniel stated that he plans to propose
similar hands-on activities for SLAC’s Safety Fair.
Thomas Jefferson Site Office
Mary Jo Bailey described the JLab’s training/work safety program that provides 5 or 6 short videos to
workers pre-work to raise hazard awareness and enhance Work Planning and Safety.
EXTERNAL ORGANIZATIONS OPERATING EXPERIENCE
AND LESSONS LEARNED DISCUSSION
HS-24 External Operating Experience Update – John (Larry) Stirling is a new HS-24 employee and will be
working on external operating experience documents. Please contact Larry Stirling to discuss possible external
operating experience products.
UPDATE ON OFFICE OF ANALYSIS AND CORPORATE
OPERATING EXPERIENCE INITIATIVES

ORPS Order and OE Order implementation- Ruocco

EFCOG’s ORPS Task Group and HSS are working on DOE O 232.2, Occurrence Reporting and
Processing of Operations Information, Responses to Frequently Asked Questions. In January, the
draft went through a final EFCOG ORPS Task Group Review, the updated document will be sent
to HS-24 management in mid-February.

One-Year Accuracy Review of DOE Order 232.2, Occurrence Reporting and Processing of
Operations Information- HS-24 has created a schedule for the review of DOE O 232.2. The
review of DOE O 232.2 for Responses the Frequently Asked Questions has facilitated the
beginning of the One Year Accuracy Review. HS-24 formally engaged the ORPS Task Group and
selected HSS and DOE Line Program representatives as the reviewing audience, specifically
requesting any comments or suggestions on DOE O 232.2 in January 2013. Ruocco is compiling
and reviewing these suggestions with HS-24 Management.
 One Year Accuracy Review of DOE Order 210.2A, DOE Corporate Operating Experience
Program- The outcome of the one-year accuracy review for DOE O 210.2A revealed that minor
editorial errors found within the Order do not warrant a Page Change at this time. The minor
grammatical errors do not hinder the use or understanding of the Order. A memo on this One-Year
Accuracy review is within the HSS approval process.

OE Wiki - Ruocco
 OE Wiki Videos- the latest video posted was “Brookhaven’s Safety Culture” from Brookhaven
National Laboratory. Other videos posted throughout the month of January include: “Deadly
Contract” from the US Chemical Safety Board, “The Flex Solution: America’s Nuclear Industry
Responds to Fukushima’ from the Nuclear Energy Institute. To access the Videos:

 Occupational Health & Safety E-News recently published an article, 3-D Meets Safety at LANL,
describing a new system for assessing glovebox designs in order to mitigate or prevent
ergonomic injuries. Users are able to create virtual models that accurately represent both genders
and that factor in human measurements, such as height, weight, and reach. Read more at

 Trang Ha, LSO, provided further information on the OE Wiki Video- Continuous Chest
Compression CPR. The Medical Director at the LLNL Health Clinic states that while the hands
only technique is legitimate and recommended, it is not clear that hands only CPR is intended
primarily for WITNESSED arrest/collapse. CPR with breaths should be used for people who are
found unconscious as well as for infants and children, and the other situations noted below.
 From the American Heart Association:
Are there times when I should use conventional CPR with breaths?
Yes. There are many medical emergencies that cause a person to be unresponsive
and to stop breathing normally. In those emergencies, conventional CPR that
includes mouth-to-mouth breathing may provide more benefit than Hands-Only
CPR. The American Heart Association recommends CPR with a combination of
breaths and compressions for:
o All infants (up to age 1)
o Children (up to puberty)
o Anyone found already unconscious and not breathing normally
o Any victims of drowning, drug overdose, collapse due to breathing
problems, or prolonged cardiac arrest

Self-Assessment reviews on the effectiveness of the OE Program- Ruocco
 Ruocco reviewed the results of the Corporate OE Self Assessment conducted by HS-24 that were
sent to DNFSB in December 2012:

Continually improve performance trending and analysis of information OE; develop a greater number of
OE documents as appropriate, to include a greater focus on nuclear safety OE documents.
Improve the process to evaluate, publish, and disseminate operating experience from external sources
(e.g., other Federal agencies; international sources).
Formally verify existing, and identify new, OE Program Coordinators; ensure OE Coordinators are aware
of their responsibilities for implementing OE requirements contained in DOE O 210.2A.

Seek information regarding, as part of self-assessments conducted to evaluate organizational performance
in ISM, Line Program and contractor assessment of the effectiveness of the organization’s OE program.
Continue to improve the productivity of the OE Committee; the conduct and quality of monthly OE
Committee meetings; and the engagement of appropriate site subject matter experts in sharing and
discussing operating experience from site, national, and international sources.
Improve the Corporate Lessons Learned Database, and the quantity and quality of lessons learned
submitted and maintained in the database.
The ‘Observations from the Office of Analysis Self Assessment of the Corporate Operating Experience
Program and General Path Forward for Continuous Improvement’ December 2012 Document is located on
the OE Wiki under February’s OEC Call and was emailed with the minutes of this meeting.
Ruocco received a lot of feedback and update to the OE Committee roster; all of the input is much
appreciated. Ruocco sent an updated OE Committee Roster to the OEC on February 8. If you have
any updates, please continue to send them to Ruocco. A few Program Coordinators have changed
positions or retired, however Ruocco has not received the names or information for the
replacements. A few missing Program Coordinators include: EM’s West Valley Demonstration
Project, FE PSO (HQ), NA’s Kansas City Site Office, NA’s Service Center, SC’s Ames Site Office.
Ruocco plans to send a current OEC Roster to the OEC on a regular basis to ensure its accuracy (at
least annually).
Please provide information on your OE Self Assessments on the OEC Calls or Meeting.
Please continue providing feedback on OE Program improvements.

Lessons Learned Program and Database- Ruocco
 Reminder for Lessons Learned Report originators to please include their area code in the Lessons
Learned document. Also, please include your email address which will assist in the Lessons
Learned Database Manager to contact originators when needed.

HQ Lessons Learned Database improvements/upgrades are being assessed. In January, HS-24
formed the Lessons Learned Process Improvement Team (composed of OE Committee and
Lessons Learned member). In February, the Lessons Learned Process Improvement Team will
review PNNL’s Lessons Learned System and BNL’s Lessons Learned System. Webinars with
both PNNL and BNL will be held. HS-24 and the Lessons Learned Process Improvement Team
will continue reviewing the HQ Lessons Learned Database and other Databases that are currently
being implemented.
Please contact Ruocco if you can give a briefing on your Lessons Learned Internal Program
and/or provide Site Lesson Learned Program/Database information to the Lessons Learned
Process Improvement Team.

The OEC plans to hold the OEC Meeting during EFCOG’s ISM & QA Spring Meeting at Argonne from
April 30 to May 2, 2013. The OEC Meeting will be a one day meeting either on May 1 or May 2. Final
date will be sent once Ruocco has confirmed with EFCOG.
