RESOLUTION NO. 2-20140102 RESOLUTION - APPOINTING PUBLIC DEFENDER POWERS, MCCARTY, ROSENTHAL RESOLVED, that pursuant to Article 18-A of the County Law, Richard A. RESOLUTION NO. 6-20140102 RESOLUTION - AUTHORIZING CHAIR TO EXECUTE CONTRACT FOR RECOVERY SUPPORT SERVICES FOR ADDICTION RECOVERY SERVICES LINDBERG, SCHWERD, KOUTNIK, ROSENTHAL WHEREAS, there is a need for recovery support services at the Otsego County Addiction Recovery Services; and WHEREAS, Friends of Recovery of Delaware and Otsego (FOR-DO), 22 Elm Street, Oneonta, New York 13820 has agreed to provide such services as set forth below; now, therefore, be it RESOLVED, that contingent upon Otsego County’s receipt of funding from New York State, the Chair of this Board be and hereby is authorized to execute a contract with FOR-DO for recovery support services for the Addiction Recovery 5 Services, commencing January 1, 2014 through December 31, 2014, exceed $151,500 for the term of the contract; and be it further not to RESOLVED, that said contract shall contain such other terms and provisions as are in the best interests of the County of Otsego; and be it further RESOLVED, that the funds for this contract are included in the 2014 Turning Point Budget (4327-A) at line 4800 Turning Point and consist of 100% reimbursement from state funds. RESOLUTION NO. 7-20140102 RESOLUTION - APPROVING EXECUTION OF CONTRACT FOR THE PROVISION OF ALCOHOLISM SERVICES IN OTSEGO COUNTY LINDBERG, SCHWERD, KOUTNIK, ROSENTHAL WHEREAS, funds are available through the New York State Office of Alcoholism and Substance Abuse Services to Otsego County for purposes of providing educational services, information and referral services for residents of Otsego County suffering from the diseases of alcoholism and other addictions; and WHEREAS, in addition, Task Force on Integrated Projects (TFIP) 100% funds are available under the auspices of the New York State Office of Alcoholism and Substance Abuse Services; these TFIP funds will be administered under the Division of Alcoholism and Alcohol Abuse (DAAA); and WHEREAS, the amount of the funding for the Information and Referral Services and the Task Force on Integrated Projects totals $412,348; and WHEREAS, the Leatherstocking Education on Alcoholism/Addictions Foundation, Inc. (LEAF, Inc.) has expressed a willingness and desire to continue to act as an agency to the County for purposes of providing these alcoholism and other addictions educational services; and WHEREAS, it appears to be in the best interests of the residents of the County of Otsego to provide these services; now, therefore, be it RESOLVED, that upon receipt of the $412,348 in funds from the State of New York as above set forth, the Chair of the Board of Representatives and the Director of the Community Services are authorized to execute an alcoholism/addictions services contract, on behalf of the County of Otsego, 6 with LEAF for the contract period January 1, 2014 through December 31, 2014; and be it further RESOLVED, that said contract shall contain such terms and provisions as are in the best interests of the County of Otsego and are complying with all federal, state and local statutes, rules and regulations which would enable LEAF to continue to utilize designated funds to operate a program of alcoholism/addictions informational and educational services in the County of Otsego; and be it further RESOLVED, that upon the expiration of the term of the aforementioned contract, a subsequent resolution must be adopted by this Board to authorize the extension of additional terms of said contract; and be it further RESOLVED, that the funds for this contract are included in the 2014 Contr Chem Dependency Program Budget (4323-A) at line 4698 LEAF and consist of 100% state funds. RESOLUTION NO. 8-20140102 RESOLUTION – AUTHORIZING THE CHAIR TO EXECUTE AN AGREEMENT WITH CATHOLIC CHARITIES OF DELAWARE AND OTSEGO COUNTIES FOR OPERATION OF THE ALTERNATIVES TO INCARCERATION PROGRAM POWERS, MCCARTY, ROSENTHAL WHEREAS, in order to assist Otsego County Courts and Law Enforcement agencies in promoting appropriate and effective Alternatives to Incarceration experiences for eligible participants, Catholic Charities of Delaware and Otsego Counties has submitted a proposal to the County for the operation of the Otsego County Alternatives to Incarceration Program (ATI); and WHEREAS, the Public Safety and Legal Affairs Committee and the Administration Committee of this Board have approved this proposal and consider it to be in the best interests of the County; now, therefore, be it RESOLVED, that the Chair of this Board is authorized to execute an agreement with Catholic Charities of Delaware and Otsego Counties, with offices located at 176 Main Street, Oneonta, New York 13820, for the operation of the Otsego County Alternatives to Incarceration Program for the year 2014, at a total cost of $25,180, payable in twelve (12) monthly installments of $2,098 per month; and be it further 7 RESOLVED, that the agreement shall contain such other terms and provisions as are in the best interest of the County of Otsego; and be it further RESOLVED, that the funding for this contract is included in the 2014 Alternatives to Incarceration Budget (3157-A) at line 4800 Other. RESOLUTION NO. 9-20140102 RESOLUTION - AUTHORIZING THE CHAIR OF THE BOARD TO EXECUTE AN AGREEMENT WITH CORNELL COOPERATIVE EXTENSION SCHOHARIE & OTSEGO COUNTIES LINDBERG, SCHWERD, KOUTNIK, ROSENTHAL WHEREAS, the County of Otsego has allocated in its budget for the fiscal year 2014 the expenditure of $125,000 for the support and maintenance of the work of the Cornell Cooperative Extension Schoharie & Otsego Counties, and for the conduct of the extended educational programs of the New York State College of Agriculture and Life Sciences, the College of Human Ecology, and other units at Cornell University; and WHEREAS, it is appropriate that a contract with Cornell Cooperative Extension Schoharie & Otsego Counties be authorized to assure proper application of this expenditure; now, therefore, be it RESOLVED, that the Chair of this Board be and hereby is authorized to contract with the Cornell Cooperative Extension Schoharie & Otsego Counties for the purpose of providing funds, not to exceed $125,000, to the Cornell Cooperative Extension Schoharie & Otsego Counties to pay towards the support and maintenance of the work of the Cooperative Extension and for the conduct of the extended educational programs of the New York State College of Agriculture and Life Sciences, the College of Human Ecology, and other units at Cornell University, for the contract term January 1, 2014 to December 31, 2014; and be it further RESOLVED, that said contract shall contain such other terms and provisions as are in the best interests of the County of Otsego; and be it further RESOLVED, that the funding for this contract is included in the 2014 Extension Association Budget (8751-A) at line 4680 Annual Support Agreement. RESOLUTION NO. 10-20140102 8 RESOLUTION - AUTHORIZING CHAIR OF THE BOARD TO EXECUTE AN EXTENSION OF THE SERVICE AGREEMENT FOR FIELD MAINTENANCE OF THE ESTS RADIO PROJECT POWERS, MCCARTY, ROSENTHAL RESOLVED, that this Board of Representatives hereby authorizes the extension of the service agreement for the field maintenance of Otsego County’s ESTS radio project for the year 2014, as follows: ITEM: Field Maintenance of the ESTS Radio Project COST: Not to exceed $25,000 VENDOR: LBJ Services 3621 County Highway 11 Cooperstown, New York 13326 TERM: January 1, 2014 through December 31, 2014 and be it further RESOLVED, that the Chair of the Board is authorized to execute the extension of the above Service Agreement; said agreement to contain such other terms and provisions as are in the best interest of the County of Otsego; and be it further RESOLVED, that the funding for this agreement is included in the 2014 Public Safety Communications Budget (3020-A) at line 2750 E-911 Communication Backbone. RESOLUTION NO. 11-20140102 RESOLUTION – AMENDING RESOLUTION NO. 321-20130904 AUTHORIZING CHAIR OF THE BOARD TO EXECUTE AGREEMENT WITH ENGINEERS FOR CONSTRUCTION INSPECTION OF BRIDGE BIN 3353930, COUNTY ROUTE 11C OVER SUSQUEHANNA RIVER, TOWNS OF HARTWICK/MIDDLEFIELD, OTSEGO COUNTY 9 MCCARTY, FRAZIER WHEREAS, Resolution No. 321-20130904 authorized the Chair of the Board to enter into an agreement with Shumaker Consulting Engineering and Land Surveying, P.C., for the construction inspection of bridge BIN 3353930, located on County Route 11C over the Susquehanna River, in the Towns of Hartwick/Middlefield, Otsego County, New York, at a cost not to exceed $47,500; and WHEREAS, there is a need to amend Resolution No. 321-2013 for an additional cost not to exceed $20,000; now, therefore, be it RESOLVED, that the Chair of the Board be and hereby is authorized to amend an agreement with Shumaker Consulting Engineering and Land Surveying, P.C., 143 Court Street, Binghamton, New York 13901, for the construction inspection of bridge BIN 3353930, located on County Route 11C over the Susquehanna River, in the Towns of Hartwick/Middlefield, Otsego County, New York, for an additional cost not to exceed $20,000 for construction inspection; and be it further RESOLVED, that said agreement shall contain such other terms and conditions as are in the best interest of the County of Otsego; and be it further RESOLVED, that the funds for this project are available in the 2013-2014 Highway Budget under D5112.2020, D5113.2121 and D5113.2104. RESOLUTION NO. 12-20140102 RESOLUTION - AUTHORIZING CHAIR OF THE BOARD TO ENTER INTO AGREEMENTS FOR PROFESSIONAL ENGINEERING SERVICES FOR VARIOUS COUNTY ROADS AND BRIDGES FOR THE YEAR 2014 MCCARTY, FRAZIER WHEREAS, certain engineering firms propose to render professional engineering services to the Otsego County Highway Department in connection with various County roads and bridges for the year 2014; and WHEREAS, it appears to be in the best interests of the County to contract with these engineering firms for these professional engineering services; said services being professional in nature and not requiring public bidding; now, therefore, be it RESOLVED, that the Chair of this Board be and hereby is authorized to contract with the following engineering firms to render professional engineering 10 services for various County roads and bridges during the year 2014, in accordance with the hourly rate schedules, which are on file in the Office of the Clerk of the Board; these services being professional in nature and not requiring public bidding by the County: 1. RESOLUTION NO. 13-20140102 RESOLUTION - AUTHORIZING CHAIR OF THE BOARD TO CONTRACT WITH BASSETT HEALTHCARE FOR MEDICAL SERVICES AT THE COUNTY CORRECTIONAL FACILITY POWERS, MCCARTY, ROSENTHAL WHEREAS, there is a need for medical services at the County's Correctional Facility; and WHEREAS, Bassett Healthcare has submitted a proposal to the County for said services; now, therefore, be it RESOLVED, that the Chair of this Board be and hereby is authorized to enter into a contract with Bassett Healthcare at a cost of $45,000 for the period January 1, 2014 through December 31, 2014 for the following: 1. A physician in the employ of Bassett Healthcare to supervise Hospital’s provision of specified medical care pursuant to Section 7010.1 of Title 9, Subtitle AA, Chapter I of the Official Compilation of Codes, Rules and Regulations of the State of New York. 2. The Hospital’s Physician will provide on-site services at the Otsego County Jail once weekly and will review the nurse’s activities and medical record documentation to meet all jail 12 inmate population needs that can be addressed on an outpatient basis. 3. The Hospital’s Physician will write prescriptions as medically necessary, but will not store drugs nor dispense drugs to inmates. 4. Hospital will provide laboratory and radiology services and inpatient and outpatient services as ordered by Physician, to be billed at the Hospital’s usual rate and to be paid by County within 30 days of receipt of invoices. and be it further RESOLVED, that said contract shall contain such other terms and provisions as are in the best interests of the County of Otsego; and be it further RESOLVED, that the funding for this contract is included in the 2014 Jail Budget (3150-A) at line 4900 Contracts. RESOLUTION NO. 14-20140102 RESOLUTION - AUTHORIZING CHAIR TO EXECUTE CONTRACT WITH PSYCHIATRIC CONSULTANT FOR SERVICES AT THE COMMUNITY SERVICES CLINIC LINDBERG, SCHWERD, KOUTNIK, ROSENTHAL WHEREAS, there is a need for the services of a Psychiatric Consultant at the Otsego County Community Services Clinic; and WHEREAS, Samuel Press, M.D. has expressed a willingness to provide these services approximately every other week to said clinic to work with children; now, therefore, be it RESOLVED, that the Chair of this Board be and hereby is authorized to execute a contract with Samuel Press, M.D., 303 Kinderhook Lane, Nassau, New York 12123, for the aforesaid services, commencing January 1, 2014 through December 31, 2014, at the rate of $160.00 per hour, not to exceed 700 hours per year, at a cost not to exceed $112,000 for the term of the contract; and be it further RESOLVED, that the services provided by Dr. Press are to be arranged by the Clinic Director; and be it further 13 RESOLVED, that Dr. Press agrees to maintain malpractice/professional liability insurance during the term of the contract; and be it further RESOLVED, that said contract shall contain such other terms and provisions as are in the best interests of the County of Otsego; and be it further RESOLVED, that the funds for this contract are included in the 2014 Mental Health Budget (4310-A) at line 4880 Mental Health Consultant and consist of 100% reimbursement from fees, state and federal funds. RESOLUTION NO. 15-20140102 RESOLUTION - AUTHORIZING CHAIR TO EXECUTE CONTRACT WITH KATE BOLGER, NPP, APRN, FOR PSYCHIATRIC NURSE PRACTITIONER SERVICES AT THE OTSEGO COUNTY COMMUNITY SERVICES CLINICS LINDBERG, SCHWERD, KOUTNIK, ROSENTHAL WHEREAS, there is a need for the services of a Psychiatric Nurse Practitioner at the Otsego County Community Services Clinics; and WHEREAS, Kate Bolger, NPP, APRN, has expressed a willingness to provide such services; now, therefore, be it RESOLVED, that the Chair of this Board be and hereby is authorized to execute a contract with Kate Bolger, NPP, APRN, 2410 Walley Road, Franklin, New York 13775, for the services of a Psychiatric Nurse Practitioner at the Otsego County Community Services Clinics, for the period January 1, 2014 through December 31, 2014, for up to 624 hours of such services at $80.00 per hour, at a cost not to exceed $49,920 for the term of the contract; and be it further RESOLVED, that said contract shall contain such other terms and provisions as are in the best interests of the County of Otsego; and be it further RESOLVED, that the funds for this contract are included in the 2014 Mental Health Budget (4310-A) at line 4880 Mental Health Consultant and consist of 100% reimbursement from fees, state and federal funds. RESOLUTION NO. 16-20140102 RESOLUTION – AUTHORIZING CHAIR TO EXECUTE CONTRACT WITH LISA CASLER, M.S., PMHNP-BCC, NPP, 14 FOR MEDICAL CONSULTATION SERVICES FOR OTSEGO COUNTY COMMUNITY SERVICES LINDBERG, SCHWERD, KOUTNIK, ROSENTHAL WHEREAS, there is a need for medical consultation services at the Otsego County Community Services Clinic; and WHEREAS, Lisa Casler, M.S., PMHNP-BCC, NPP, has expressed a willingness to provide such services; now, therefore, be it RESOLVED, that the Chair of this Board be and hereby is authorized to execute a contract with Lisa Casler, M.S., PMHNP-BCC, NPP, 32 Hartford Terrace, New Hartford, New York 13413, for medical consultation services for the Otsego County Community Services Clinic, for the period January 1, 2014 through December 31, 2014, at the rate of $80.00 per hour, for up to 800 hours, not to exceed $64,000 for the term of the contract; and be it further RESOLVED, that said contract shall contain such other terms and provisions as are in the best interest of the County of Otsego; and be it further RESOLVED, that the funds for this contract are included in the 2014 Mental Health Budget (4310-A) at line 4880 Mental Health Consultant and consist of 100% reimbursement from fees, state and federal aid/reimbursement. RESOLUTION NO. 18-20140102 RESOLUTION – AUTHORIZING CHAIR OF THE BOARD TO CONTRACT WITH THE MENTAL HEALTH ASSOCIATION IN ULSTER COUNTY, INC. LINDBERG, SCHWERD, KOUTNIK, ROSENTHAL WHEREAS, the State of New York has provided a grant in the amount of $197,032 through the Community Reinvestment Act, for services of a Psychiatric Nurse Practitioner or other qualified medical provider, to operate Adolescent Outreach and Intensive Prevention Services; and WHEREAS, the County, through the Otsego County Community Services Board, desires to contract with the Mental Health Association in Ulster County, Inc., 221 Tuytenbridge Road, Lake Katrine, New York 12449, to effectuate these services for the year 2014; the same to be without cost to the County other than this state grant; now, therefore, be it RESOLVED, that the Chair of this Board is authorized to execute a contract for the year 2014 with the Mental Health Association in Ulster County, Inc. for the services of a Psychiatric Nurse Practitioner or other qualified medical provider, to operate Adolescent Outreach and Intensive Preventive Services for children and adolescents aged 5-21 years, utilizing 100% Community Reinvestment Act funding in the amount not to exceed $197,032, contingent upon the availability of on-going state funding; and be it further RESOLVED, that said contract shall contain such other terms and obligations as shall be in the best interest of the County of Otsego; and be it 16 further RESOLVED, that funding for this contract is included in the 2014 Harbour Program – Reinvestment Budget (4326-A) at line 4693 MHA and is 100% reimbursable through New York State Community Reinvestment Act Funding. RESOLUTION NO. 19-20140102 RESOLUTION – AUTHORIZING CHAIR OF THE BOARD TO CONTRACT WITH THE FAMILY RESOURCE NETWORK, INC. FOR FAMILY SUPPORT SERVICES LINDBERG, SCHWERD, KOUTNIK, ROSENTHAL WHEREAS, funding through the New York State Community Reinvestment Act in the amount of $25,188 is available for providing a Coordinated Children’s Services Initiative Coordinator for the County of Otsego; specifically, providing service coordination to parents and families with mental health needs, who are struggling with access to services and supports due to their children’s cross system needs, with the support and skills necessary to promote positive outcomes for their children, aged birth through 21 years; and WHEREAS, the County, through the Otsego County Community Services Board, desires to contract with The Family Resource Network, Inc., to effectuate these services from January 1, 2014 through December 31, 2014 utilizing this state grant totaling $25,188; now, therefore be it RESOLVED, that the Chair of this Board be and hereby is authorized to execute a contract with The Family Resource Network, Inc. to provide a Coordinated Children’s Services Initiative Coordinator for the County of Otsego for the period of January 1, 2014 through December 31, 2014 utilizing this state grant totaling $25,188; and be it further RESOLVED, that said contract shall contain such other terms and obligations as shall be in the best interest of the County of Otsego; and be it further RESOLVED, that the funding for this contract is included in the 2014 Coordinated Childrens Initiative Budget (4329-A) at line 4694 Family Resource Network and 100% reimbursable through New York State Community Reinvestment Act Funding. RESOLUTION NO. 20-20140102 17 RESOLUTION – AUTHORIZING THE CHAIR OF THE BOARD TO ENTER INTO AN AGREEMENT WITH RESOURCE CENTER FOR INDEPENDENT LIVING FOR THE PROVISION OF VETERANS DIRECTED HOME AND COMMUNITY BASED SERVICES THROUGH THE OFFICE FOR THE AGING LINDBERG, SCHWERD, KOUTNIK, ROSENTHAL WHEREAS, the Veterans Administration has identified a need for Veterans Directed Home and Community Based Services in Otsego County; and WHEREAS, Resolution 77-20110209 authorized the Chairman of the Board to enter into a Provider Agreement with the Veteran’s Administration to provide such services through the Office for the Aging; and WHEREAS, said services require a Fiscal Intermediary to act as employer of record for Veteran Directed Workers, and Resource Center for Independent Living of 409 Columbia Street, PO Box 210, Utica NY 13503 is willing and able to provide this service; and WHEREAS, all services are determined appropriate through the completion of an assessment, care plan and an individual budget approved by the Veterans Administration; and WHEREAS, such reimbursement shall be approximately $66,000; now, therefore, be it RESOLVED, that the Chair of this Board be and hereby is authorized to execute an agreement with Resource Center for Independent Living to provide such services at an amount not to exceed $66,000 for the period of January 1, 2014 through December 31, 2014; and be it further RESOLVED, that these funds are included in the 2014 Office for the Aging Budget, and are 100% reimbursable. RESOLUTION NO. 21-20140102 RESOLUTION – AUTHORIZING RETAINING A CONSULTANT DIETICIAN BY THE OFFICE FOR THE AGING LINDBERG, SCHWERD, KOUTNIK, ROSENTHAL WHEREAS, this Board has heretofore approved the administration of the 18 Title III-C and the Supplemental Nutrition Assistance Program; and within each of the above programs, there is a requirement of a position of Consultant Dietician; and WHEREAS, Title IIID of the Older Americans Act requires provision of Health Promotional Activities; and WHEREAS, Regina M. La Tourette has expressed a willingness to provide these services under these programs; and WHEREAS, the Nutrition Program Standards require that this Consultant Dietician report directly to the Otsego County Office for the Aging for both nutrition programs; now, therefore, be it RESOLVED, that the Chair of the Board be and is hereby authorized to enter into an agreement with Regina M. RESOLUTION NO. 28-20140102 RESOLUTION - AUTHORIZING OTSEGO COUNTY COMMISSIONER OF SOCIAL SERVICES TO CONTRACT WITH OTSEGO COUNTY COMMUNITY SERVICES KOUTNIK WHEREAS, the County has been awarded Flexible Funding for Family Services in the overall sum of $2,411,179 for 2013 - 2014; and WHEREAS, the New York State Office of Temporary and Disability 25 Assistance has approved the County’s plan for the use of these funds; and WHEREAS, the County desires to utilize a portion of the funds, not to exceed $61,416 to purchase existing services for TANF eligible applicants and recipients in need of mental health services; and WHEREAS, the County desires to utilize existing programming to obtain services for eligible families; and WHEREAS, the Department desires to enter into an agreement with the Otsego County Community Services for the services of two (2) part-time Mental Health Social Workers; now, therefore, be it RESOLVED, that the Otsego County Commissioner of Social Services is hereby authorized to execute an agreement with Otsego County Community Services for the term January 1, 2014 through December 31, 2014; and that the agreement shall include the following terms: 1. RESOLUTION NO. 29-20140102 RESOLUTION – AUTHORIZING COMMISSIONER OF SOCIAL SERVICES TO RENEW CONTRACT WITH DISTRICT ATTORNEY FOR WELFARE FRAUD INVESTIGATION AND PART-TIME ADA SERVICES KOUTNIK RESOLVED, that the Otsego County Commissioner of Social Services and the Otsego County District Attorney are hereby authorized to contract for welfare fraud investigation services through the District Attorney’s Office; the contract will provide three (3) part-time investigators, and part-time ADA services by hourly rate, at a cost not to exceed $91,467 for the period January 1, 2014 through December 31, 2014; and be it further 26 RESOLVED, that said contract is to contain such other terms and conditions as are in the best interests of the County of Otsego; and be it further RESOLVED, that the funding for this contract is included in the 2014 Social Services Administration Budget (6010-A) at line 4900 Contracts and is 33% Federal and 5% State reimbursable and 62% Local cost. RESOLUTION NO. 30-20140102 RESOLUTION - AUTHORIZING COMMISSIONER OF SOCIAL SERVICES AND THE DIRECTOR OF OFFICE FOR THE AGING TO EXECUTE AN AGREEMENT FOR VARIOUS MEDICAID FUNDED LONG TERM CARE IN-HOME SERVICES PROGRAMS AND THE OPERATION OF A POINT OF ENTRY FOR LONG TERM CARE SERVICES KOUTNIK WHEREAS, Social Services has a responsibility to administer and case manage various Medicaid funded long term care in-home service programs including the Personal Care Aide Program, the Long Term Home Health Care Program, Personal Emergency Response Systems, the Consumer Directed Personal Assistance Program, the Assisted Living Program, the Care at Home Program, Private Duty Nursing and the Title XX Family Type Homes for Adult Supervision, and has a long-standing history of providing information and assistance to county residents regarding long term care services; and WHEREAS, the Otsego County Office for the Aging has expressed a willingness to provide the administration and case management of these and any such successor or related programs; now, therefore, be it RESOLVED, that the Commissioner of Social Services and the Director of the Otsego County Office for the Aging are hereby authorized to execute an agreement and other documents that may be necessary for the provision of these services in an amount not to exceed $273,154 for the period of January 1, 2014 through December 31, 2014; and be it further RESOLVED, that the funding for this contract is included in the 2014 Social Services Administration Budget (6010-A) at line 4900 Contracts and is 95% Federal and State reimbursable and 5% local costs. RESOLUTION NO. 31-20140102 RESOLUTION - AUTHORIZING OTSEGO COUNTY 27 COMMISSIONER OF SOCIAL SERVICES TO CONTRACT WITH SEVERAL AREA SCHOOL DISTRICTS KOUTNIK WHEREAS, the Otsego County Department of Social Services has determined that the interests of the students and their families located in the Otsego County school districts will be best served by maintaining a Caseworker within each school district staffed by an Otsego County Department of Social Services caseworker(s); and WHEREAS, the administration of each school district has proposed to contract with the County for the services of a caseworker(s) to perform preventive services for the students and their families within their respective school districts; and WHEREAS, in addition to reimbursing the County in full for the local share of the salary of a caseworker(s), the administration of each school district has proposed to pay for the overhead and other benefits and costs of maintaining such position(s); and WHEREAS, it is in the best interest of the County of Otsego to contract for position(s) with each school district; now, therefore, be it RESOLVED, that the Otsego County Commissioner of Social Services is hereby authorized to contract with each of the following school districts for the period January 1, 2014 through December 31, 2014: Cherry Valley-Springfield School District Edmeston School District Milford School District Oneonta City School District Richfield Springs School District Unatego School District (3 Caseworkers) for preventive services, to be furnished by Otsego County, with a caseworker(s) located in each above-listed district, with 12% of the salary, benefits, costs and other additional overhead necessary for the operation of the child preventive services program being paid for by each school district; and be it further RESOLVED, that said contracts shall contain such other terms and provisions as are in the best interests of the County of Otsego; and be it further RESOLVED, that the funds for the above contracts are included in the 2014 Social Services Administration Budget (6010-A) at line 1000 Personal Service and 28 consist of 64% Federal funds, 24% State funds, and 12% school district funds. RESOLUTION NO. 32-20140102 RESOLUTION – AUTHORIZING OTSEGO COUNTY COMMISSIONER OF SOCIAL SERVICES TO RENEW CONTRACT WITH OPPORTUNITIES FOR OTSEGO, INC. (OFO) TO PROVIDE EMERGENCY HOUSING AT THE OFO HOMELESS SHELTER KOUTNIK WHEREAS, the Otsego County Department of Social Services is required by regulation to provide emergency housing to homeless individuals and families; and WHEREAS, the County desires to meet this requirement in the most cost effective manner while at the same time providing some case management services; and WHEREAS, Opportunities for Otsego, Inc. has established a bed rate of $75.00 per day per person; now, therefore, be it RESOLVED, that the Commissioner of the Department of Social Services, on behalf of the County of Otsego, is hereby authorized to renew a contract with Opportunities for Otsego, Inc. for the term January 1, 2014 through December 31, 2014; and said contract shall include the following: 1. RESOLUTION NO. 34-20140102 RESOLUTION – AUTHORIZING OTSEGO COUNTY COMMISSIONER OF SOCIAL SERVICES TO RENEW CONTRACT WITH THE OTSEGO COUNTY PROBATION DEPARTMENT KOUTNIK WHEREAS, Otsego County has been awarded Flexible Funding for Family Services in the overall sum of $2,411,179 for 2013- 2014; and WHEREAS, the New York State Office of Temporary and Disability Assistance has approved the County’s plan for the use of these funds; and WHEREAS, there is a need to provide PINS Diversion Services to the residents of Otsego County; and WHEREAS, the Otsego County Probation Department has expressed a willingness to provide this service; now, therefore be it RESOLVED, that the Otsego County Commissioner of Social Services is hereby authorized to contract with the Otsego County Probation Department for the services of two (2) Probation Officers to provide PINS Diversion services on 30 behalf of the Department of Social Services, at a cost not to exceed $129,463 for the period January 1, 2014 through December 31, 2014; and be it further RESOLVED, that said contract shall contain such other terms and provisions as are in the best interests of the County of Otsego; and be it further RESOLVED, that the funding for the contract is included in the 2014 Social Services Administration Budget (6010-A) at line 4900 Contracts and is 100% reimbursable through Federal funds. RESOLUTION NO. 35-20140102 RESOLUTION – AUTHORIZING COMMISSIONER OF SOCIAL SERVICES TO RENEW CONTRACT WITH MTS SOFTWARE SOLUTIONS FOR SYSTEMS SERVICE AND SUPPORT KOUTNIK WHEREAS, the Department of Social Services wishes to enter into a contract with MTS Software Solutions for support services for a document imaging system at the Meadows Office Complex and at the County Office Building located at 197 Main Street, Cooperstown, New York; and WHEREAS, such system support services are required on an ongoing basis to maintain the system; now, therefore, be it RESOLVED, that the Commissioner of the Department of Social Services, on behalf of the County of Otsego, is hereby authorized to enter into a contract with MTS Software Solutions, 225 Executive Drive, Suite 4, Moorestown, New Jersey, 08057 for the purpose of providing ongoing systems service and support for the period of January 1, 2014 through December 31, 2014; said contract shall include the following terms: 1. In no event shall the Department of Social Services’ liability exceed the sum of $ 28,185 for the term of the contract; 2. The contract shall contain such other and further conditions as are in the best interest of the County of Otsego; and be it further RESOLVED, that the funding for this contract is included in the 2014 Social Services Records Management Budget (6011-A) at line 4900 Contracts and consists of 66% federal, 16% state reimbursement and 18% local cost. 31 RESOLUTION NO. 37-20140102 RESOLUTION – AUTHORIZING THE CHAIR OF THE BOARD TO EXECUTE AN AGREEMENT WITH OTSEGO COUNTY SOIL & WATER CONSERVATION DISTRICT (OCSWCD) TO IMPLEMENT A PILOT COMPOSTING PROGRAM ROWINSKI, POWERS, MCCARTY WHEREAS, Resolution #111-20120307 engaged Otsego County in an 18 month contract agreement with the Otsego County Soil & Water Conservation District to implement a Pilot Composting Program; and WHEREAS, Otsego County allocated in 2012 a maximum amount of $30,000 for an initial term of eighteen months; WHEREAS, the OCSWCD has launched the compost pilot project in 2012 and 2013 and now produces over eight tons of composted kitchen waste from County buildings; and WHEREAS, the Otsego County Compost Project has kept waste from regional landfills and reduced County operating costs; and WHEREAS, the Otsego County Compost Project has received a seal of approval from Cornell University and has been deemed successful; and WHEREAS, the existing contract with the OCSWCD ends December 31, 2013; and WHEREAS, the Otsego SWCD Pilot Composting Program as of September 10, 2013 has expended $19,140.10; and WHEREAS, it is in the county's best interest to continue to support the county-wide compost pilot project; now therefore, be it RESOLVED, that the Chair of the Board is authorized to execute an extension of the contract agreement with Otsego County Soil and Water Conservation District, for a period of one (1) year to continue the county-wide compost pilot program at a cost not to exceed $10,859.90 for the term of the contract extension, as described in the project/budget summary for 2014 and on file in the Office of the Clerk of the Board; and be it further RESOLVED, that the funds for this Contract agreement extension are 32 included in the 2014 Solid Waste Budget under line item 8160-4900-A, Contracts and shall be funded by the Solid Waste User Fee. RESOLUTION NO. 38-20140102 RESOLUTION – AUTHORIZING CHAIR OF THE BOARD TO EXTEND AN AGREEMENT WITH OTSEGO COUNTY CHAPTER NYSARC, INC. (HEREAFTER REFERRED TO AS ARC OTSEGO) FOR EXPANSION OF THE OTSEGO COUNTY PUBLIC TRANSPORTATION SYSTEM SCHWERD, ROWINSKI, FRAZIER WHEREAS, Otsego County, under a partnership with Arc Otsego, entered into an agreement for 2012 to open Arc Otsego’s current bus route, referred to as Route 9, Hartwick, Garrattsville, Morris, Gilbertsville to Oneonta, for public use; and WHEREAS, Arc Otsego expressed an interest to continue the agreement for 2014 and to coordinate this route with Otsego County to open the route to the general public in order to service this area of the county; now, therefore, be it RESOLVED, that the Chair of this Board is authorized to extend an agreement with Arc Otsego for the year 2014 to offer public transportation service Monday through Friday along a fixed route beginning in Hartwick-New Lisbon-Garrattsville-Morris-Gilbertsville-Otego and Oneonta with a reverse trip in the afternoon, with a cost share to Otsego County calculated at $1,721.33 per month ($20,656 per year) based on a 60% (Arc based clients) and 40% (general public) relationship of anticipated ridership; and be it further RESOLVED, that the agreement shall include quarterly reviews of ridership to monitor the cost share arrangement which will be adjusted to accurately reflect the percentage of Arc versus non-Arc riders; and be it further RESOLVED, that the County’s share will be allocated through State Transportation Operating Assistance (STOA) and New York State Department of Transportation 5311 operating funding and is included in the 2014 Public Transportation Service Budget (5630-A) at line 4900 Contracts. RESOLUTION NO. 39-20140102 RESOLUTION - AUTHORIZING CHAIR TO EXECUTE 33 CONTRACT WITH REGISTERED NURSE FOR SERVICES AT ADDICTION RECOVERY SERVICES LINDBERG, SCHWERD, KOUTNIK, ROSENTHAL WHEREAS, there is a need for the services of a Registered Nurse at Addiction Recovery Services; and WHEREAS, Christine Mitchell, RN, has expressed a willingness to provide these services approximately every week to said clinic; now, therefore, be it RESOLVED, that the Chair of this Board be and hereby is authorized to execute a contract with Christine Mitchell, RN, 473 Evening Inn Road, Oneonta, New York 13820, for services to Addiction Recovery Services; said services to include medical and drug monitoring and state-required health assessments, commencing January 1, 2014 and terminating December 31, 2014, at the rate of $39.00 per hour, for 650 hours, not to exceed $25,350 during the term of the contract; and be it further RESOLVED, that said contract shall contain such other terms and provisions as are in the best interests of the County of Otsego; and be it further RESOLVED, that the funds for this contract are included in the 2014 Alcoholism Budget (4250-A) at line 4880 Consultants and consist of 100% reimbursement from fees, state and federal funds. RESOLUTION NO. 40-20140102 RESOLUTION - AUTHORIZING THE CHAIR OF THE BOARD TO EXECUTE CONTRACT WITH THE OTSEGO COUNTY CHAPTER OF NYS ARC, INC. FOR SHELTERED EMPLOYMENT LINDBERG, SCHWERD, KOUTNIK, ROSENTHAL WHEREAS, the Otsego County Chapter of NYS ARC, Inc. provides sheltered employment to the developmentally disabled population of Otsego County; and WHEREAS, the Otsego County Board of Representatives agrees that there is a need for these services and wishes to support them via pass-through state aid funds and a local government contribution; now, therefore, be it RESOLVED, that the Chair of the Board of Representatives be and hereby 34 is authorized to enter into a contract with the Otsego County Chapter of NYS ARC, Inc. for the above services, for the period January 1, 2014 through December 31, 2014, with total financial support for these services at $263,120, consisting of the following: Office of People with Developmental Disabilities County Contribution TOTAL $229,370.00 33,750.00 $263,120.00 and be it further RESOLVED, that said contract shall contain such other terms and provisions as are in the best interests of the County of Otsego; and be it further RESOLVED, that the above contract is authorized in the 2014 Association for Retarded Citizens Budget (4330-A) at line 4695 ARC. RESOLUTION NO. 41-20140102 RESOLUTION – AUTHORIZING THE CHAIR OF THE BOARD TO EXECUTE A SEVENTH AMENDMENT TO THE PRESENT CONTRACT WITH BLUE WING SERVICES, INC. FOR ADDITIONAL SERVICES POWERS, MCCARTY, ROSENTHAL WHEREAS, Blue Wing Services, Inc. has been providing project management and engineering services as well as other services to the County for the public safety communications project; and WHEREAS, the County has received several grants that must be used for project management and engineering services which currently have funds available as follows: $88,317 from the SICG Grant and $170,000 from the PSAP Grant, and which funding expires on or about April 20, 2015; and WHEREAS, the County has allocated $71,000 from the Communications Backbone for this project; and WHEREAS, it has been recommended that the County commit the funds to obtain these services; and WHEREAS, additional engineering and project management services, as well as other services, are needed from Blue Wing Services, Inc. to complete the project, thus making it necessary for the parties to execute another amendment 35 to the present contract in the amount of $329,317; now, therefore, be it RESOLVED, that the Chair of the Board is authorized to execute a seventh amendment to the present contract with Blue Wing Services, Inc., for professional project management and engineering services, and other services, at a cost not to exceed $329,317 and to extend the contract to April 20, 2015; and be it further RESOLVED, that this amendment to the present contract shall contain such other conditions as deemed in the best interests of the County; and be it further RESOLVED, that the funding for this amendment to the present contract are included in the Public Safety Communications Budget (3020-A) at line 2750 Communication Backbone, at line 4901 PSAP Grant, and at line 4905 SICG Grant.
