PURPOSE OF POSITION
Work involves establishing program goals and objectives for chronic disease prevention and
tobacco control initiatives; developing project procedures, workplan and activities; developing
schedules, priorities, and standards for achieving project goals; evaluating program activities;
submitting budget requests; and coordinating program activities. May plan, assign, and/or
supervise the work of others. Works under limited supervision of the Community
Transformation Program Manager.









ESSENTIAL FUNCTIONS
Conducts community outreach activities, meetings, and trainings focused on tobacco
control and community health initiatives.
Develops priority plans for projects and implements, coordinates, monitors, and evaluates
programs.
Works with Program Manager to establish tobacco control and health living coalitions.
Assist with media relations as it relates to the work plan and development of public service
announcements in assigned community.
Establishes program goals and objectives; develops schedule, priorities, and standards for
achieving goals; and keeps records for program evaluation activities.
Develops and implements techniques for evaluating program activities.
Prepares and submits productivity, activity, and budget reports.
May prepare and conduct seminars and presentations regarding assigned program area for
citizens, clients, staff, management, or elected officials.
Performs related work as assigned.
MINIMUM QUALIFICATIONS
Must have basic knowledge of health education, promotion, tobacco control, and/or chronic
disease prevention. Graduation from an accredited four-year college or university with major
course work in a field relevant to assignment is generally preferred. Experience and education
may be substituted for one another.
