Illinois Heart Rescue (ILHR) High School CPR Video Competition
Request for Proposals
Purpose and background of Illinois Heart Rescue:
Illinois Heart Rescue (ILHR) is a statewide quality improvement project for the State of
Illinois that is tasked with more than doubling neurologically intact survival for victims of
out of hospital cardiac arrests (OHCA). ILHR is grant funded by the Medtronic
Foundation Philanthropy.
Mission of Statewide High School CPR Video Competition
Research has demonstrated that bystander compression only CPR can double to triple
survival! It is also well documented that students from 7th-12th grade are excellent
teachers of Compression Only CPR when asked to “pay it forward” and teach their family
and friends. Unfortunately it is also true that most CPR training videos are boring and
outdated. The purpose of this competition is to have students create fun and
contemporary compression only CPR videos. These videos will be judged on a local,
regional, and State level. The top 3 videos will be posted on YouTube and the entire State
will vote. The top video will be played at major sporting events in the Chicago-land area.

Overall guidelines
Projects must focus on:
1. Improving awareness and education of sudden cardiac arrest (SCA) to help
increase rates of bystander CPR and AED use.
2. Providing innovative and/or creative methods of CPR-AED awareness and
training. It can be a skit, a song, a rap, a dance, animation, etc. Note: videos will
not be judged on quality of film but rather on content, energy and creativity.
Eligibility
 Students attending Illinois high schools
 9th-12th grade at time of submission
Video specifications
 LENGTH: The video should be no more than 90 seconds in length and 2 GB in size
Format: Popular Video Editing Applications - Macintosh
iMovie, Final Cut Pro X, Adobe Premiere Elements, Adobe Premiere CC

Basic Recording Settings
 Minimum Resolution : 1280x720 (720p)

Preferred Resolution: 1920x1080 (1080p)

Record at 24 or 30 frames per second
If using a smart phone for video recording, hold the phone in the horizontal orientation

CONTENT (must meet the standards of the American Heart Association or
American Red Cross for Compression Only CPR)
o Check for responsiveness: Are they just sleeping? Do they respond to
their name?
o Are they breathing normally? Victims of SCA often sound like they are
snoring or gasping for air like a fish out of water, or they may not be
breathing at all. (This is not normal breathing!)
o Call 911: Even if you forget how to do CPR, dispatchers will walk you
through it
o Compression Only CPR (no mouth to mouth instructions)
o Mechanics of Compression Only CPR:
 Arms straight, put one hand on top of the other
 Place your hands in the center of their chest on the breast bone,
right between the nipples
 With your shoulders over your hands, push straight down on the
chest
 The rate is 100 times per minute (it may help to pick music that has
a cadence of 100 bpm) *see Fair Use below
 Push down at least 2 inches or pretend you are pushing the chest
diameter in half
 Let the chest completely come back to normal (recoil) in between
compressions. (this allows the heart to fill back up with blood)
 Keep doing compressions until 911 arrives
 Swap out compressors every 2 minutes, or sooner, if you get tired.
 AED instructions: Send a specific person off cameral for an AED.
To eliminate the need for an AED Trainer in the video please include

the statement: “You don’t need special training to use an AED,
anyone can use it. Just turn it on and follow the voice prompts.”
ALL participants must have a signed Consent and Assignment form attached
to their submission

If copyrighted music or other works of art will be used in the production of this video
please review the Fair Use under the United States Copyright Act. It is the understanding
of ILHR that if the works of art, including music are not altered from their original form,
are not used for profit and are used for educational purposes that their use will be
protected from copyright infringement. However, each school submission should review
this act and make prudent decisions based on the information presented. Stanford
University offers interpretation and easy language for understanding the Fair Use Act in

Contest Timeline and Judging
Video submission: Submissions will be accepted DropBox up until midnight on April 15,
2015. No submissions will be accepted after this time.
Selection process using tiered judging
Each high school can create numerous videos
 Voting to occur by student body
 Submission of ONE winning video (per school)
to ILHR
Winner advances to school district
 Voting by area hospitals, regional
fire departments, and ILHR representative
Winner advances to EMS region
 Voting by EMS Medical Directors and
local business leaders
Winners advance (11, one from each EMS region)
 Regions 7-11 Chicagoland
 Regions 1-3 Rockford to Peoria
 Regions 4-6 Central and Southern Illinois
Voting by Pink Hippo Projects, Cates Collaborative,
Goodman Theatre

Prizes: To be awarded to the schools not the individual students. Prize money must
be used for “heart healthy” programs including, dietary, athletics, CPR/Health training,
etc.

1st place: $3500
2nd place: $3000
3rd place: $2500
Each EMS region winner (that is not in 1-3rd place) 8 winners: $2000 each (total
$16K)

Video Considerations
 The entry spokesperson must submit the video on behalf of his/her team and all
persons depicted in the video.
 By submitting an entry, the entry spokesperson acknowledges on behalf of his/her
team that the entry submitted may be posted on ILHR website, on YouTube, ILHR
twitter, ILHR FaceBook page, and elsewhere. ILHR has the right and discretion to
invite viewers to vote on the posted videos.
 Videos may not include unsuitable, offensive or otherwise objectionable content,
subject to ILHR’s sole discretion. (“rating” should be PG or PG 13)
 Videos may not contain or mention the name or likeness of any celebrities or
public figures, living or dead, unless written permission is obtained and is
provided at the time of entry.
 Any video that does not comply with the above requirements is subject to be
disqualified at ILHR’s sole and absolute discretion.

For schools without CPR training supplies, consider borrowing equipment from
area hospitals and/or fire departments.

PHOTOGRAPHY/VIDEO ASSIGNMENT AND CONSENT FORM (SCHOOL)
___________________________ (“School”) has elected to participate in a video project
(“Project”) for the Illinois HeartRescue Project (“IHRP”), and has agreed to submit a video
recording prepared by students enrolled at School (“the Video”) to The Board of Trustees of
the University of Illinois (“University”) on behalf of the IHRP. By signing this Assignment
and Consent Form (“Form”), School hereby agrees to the following.
School hereby represents and warrants that it has obtained from each student participant in the
Video, whether such student participated as a videographer, producer, director, actor or other
participation activity (“Participant”), an executed “PHOTOGRAPHY/VIDEO ASSIGNMENT
AND CONSENT FORM” (“Participant Form”). School hereby acknowledges and agrees that
its participation in the Project, including its eligibility for any contest related to the Project, is
contingent on any submission for the project being accompanied by this Form, executed by
School’s duly authorized representative, as well as the Participant Form(s) executed by each
Participant or (if under the age of 18) their parent or legal guardian.
School hereby agrees and acknowledges that it may not be compensated in any way for the use
of the Video, or any images or photographs, negatives, film reels, transparencies, and/or digital
files or other mediums, their content, and all copyrights or other intellectual property rights
therein shall be the sole property of University. School hereby assigns to the University,
permanently and irrevocably, all intellectual property rights (including, without limitation, all
exclusive copyrights) it has, had or may have in the Video.
School hereby releases, indemnifies, and forever discharges the University, its trustees,
officers, agents, employees, volunteers, contractors and assigns of and from any and all claims
and/or damages arising out of or in any way connected with the taking, use, sale, distribution,
publication, and or reproduction of the Video.

PHOTOGRAPHY/VIDEO ASSIGNMENT AND CONSENT FORM
Through your participation, whether as a producer, videographer, actor or other participant
(collectively “Participant”) in a video project (“Project”) for the Illinois HeartRescue Project
(“IHRP”), a video recording in which you participated or appeared (“the Video”) may be
submitted to The Board of Trustees of the University of Illinois (“University”) on behalf of the
IHRP. By signing this Assignment and Consent Form (“Form”), you are giving the University
and IHRP permission to use the Video, including your image or likeness and/or your voice, in
any format or medium, including as set forth below.
I/We, the undersigned, hereby knowingly authorize the use, sale, distribution, publication and
reproduction in perpetuity of the Video, and any and all such photographs, motion pictures,
video recordings in any medium or format now known or later invented, which have been
taken of me or those I represent for any educational or promotional purpose, including but not
limited to: (1) electronic publication on University or IHRP websites or other websites
(including, without limitation, Youtube); and (2) for use or publication in the University’s or
IHRP’s publications or newsletters, however published; and (3) for use in displays or
advertisements promoting the University or IHRP or any of their respective programs; and (4)
for any other educational or promotional purpose, publication, instructional situation or similar
endeavor.
I further understand that the University will be unable to prevent unauthorized persons from
gaining access to the Internet/World Wide Web or successor electronic medium and the
University websites and/ as such, will be unable to prevent the copying, alteration or
republication of the Video, including my image, likeness or voice. I understand and agree that
I will not be compensated in any way for the use of said images and all photographs, negatives,
film reels, transparencies, and/or digital files or other mediums, their content, and all
copyrights or other intellectual property rights therein shall be the sole property of University.
I/We, the undersigned, hereby assign to the University, permanently and irrevocably, all
intellectual property rights (including, without limitation, all exclusive copyrights) I have, had

My authorization includes the modification or retouching of any images as the University
deems necessary or appropriate.

agetns, employees, volunteers, contractors and assigns of and from any and all claims and/or
damages arising out of or in any way connected with the taking, use, sale, distribution,
