1. Contract Activity Description
A Parents as Teaches (PAT) Educator at the Jackson County Family Resource Center will utilize the National PAT Curriculum with with
children ages prenatal to five in Jackson County and their families, adhering to all PAT standards. Priority for participation will be given to
families with at least one risk factor according to PAT standards. Parents will learn to interact with their children to stimulate development
of vital skills and to access other family support resources as needed. The PAT Educator will conduct weekly, bi-weekly, or monthly home
visits according to the needs of families. The Educator will also facilitate monthly group meetings with participant families. The PAT
Educator will provide developmentally appropriate information and activities that focus on all domains of development along with
developmental screenings and referral for additional resources and Early Intervention as needed. The PAT Educator will coach caregivers in
engaging in learning activities with their children, encourage literacy, respond to parents concerns about child rearing and link families to
community resources. She will also encourage and facilitate understanding of the importance of consistent medical care. The program will
submit an annual report to Parents as Teachers as required by the National PAT office and will engage in the PAT Self-Assessment
according to National PAT recommendations.

3. Activity Outcomes
Please review your progress toward the following outcomes.
Outcome 1: Of the families who participate in 75% of the monthly visits, 85% will experience an increase in their confidence and competence
in parenting skills, as measured by responses in the Parents as Teachers Parent Survey. Survey will be conducted bi-annually.
