Physical Education
Ms. Fazekas and Mr. Vackert
1. You must dress for class everyday, unless told otherwise. (Sweat- pants,
shorts, t-shirts with sleeves, sweatshirts, …obviously something appropriate for
gym class. (Pg. 27 Letter A of the student handbook)
2. You must wear gym shoes…no black soled shoes, boots, sandals, etc.
3. ABSOLUTELY NO JEWELRY!!! (Pg. 27 Letter K of student handbook)
This includes but is not limited to, piercing(s), necklaces and bracelets.
Refusal to take out or off jewelry may result in a visit to the office.
4. No gum, candy or food of any kind.
5. You must participate. You may not like the activity we are doing, but you must
participate! This is not an option.
6. You are offered a lock (girls) for your gym locker. If you lose your lock for any
reason, the school will charge students $6.
7. Be respectful of others…this includes everyone. There will be no making fun in
this class. Phys. Ed. should be enjoyable…not stressful.
8. Respect the equipment. You will be charged for anything broken or lost due to
your actions.
9. Any disrespect in this class will be a direct trip to the office. (We are not
anticipating this to be a problem)
10. All students are to wait in the locker room until the entire class is ready to go
to the gym as a class. We can’t be in two places at once.
11. A parental/guardian note is required if you are unable to participate in the
class activities. A doctor’s note is required if you miss 3 or more days due to
the same illness or injury.
Grade Scale
Students will earn grades each day that will be used in a unit activity score
according to the following Grading Rubric …

10 pts
Student is always on task…including dressed for class

8 pts
Student has been warned at least once
Student is Generally on task
Student fails to dress for class

Multiple Warnings have been given to the student
The student is being a distraction to the class

Student has to sit out of the activity for any length

Student is a distraction to the class

Multiple warnings were given to the student and/or student
was sent to the office for insubordination
OTHER ITEMS
**The highest grade a student can earn who does not dress for class is a B
**Any student missing a graded day of class due to injury, illness or other reason
will make up that grade by completing a sport information worksheet packet.
**There will be written tests in team sports such as (softball-soccer-volleyballbasketball and misc. sports).
**Students will also be assessed with skills tests … testing their gross motor
skills with each activity. This is in regards to the Ohio Department of Education
Physical Education Benchmark Assessments for K-12.
