Below is the information needed to take the required CAPA
Survey. If you have not already done so, complete the survey by this Friday, March 24, 2006. Thank you.
Instructions on how to take the Capa Surveys
Each school is issued one login:
School and Community Survey
Go to www.edsolution.org
Click login at the lower right hand corner.
Enter the Username:
Enter the Password: school_community garfieldhs
Click on the “School House” to begin taking the survey.
In the first section of the survey, choose the appropriate “Role,” “Grade,” etc. and click continue and a there is a place to enter your name/address/phone number (optional).
Here the person taking the survey can go through some or all of the CAPA areas and answer questions.
Once the person taking the survey has completed some or all of the CAPA areas, they must click
“Submit Survey.”
Once the survey is submitted, it is ready to be taken again. If no one else needs t o take the survey, click “Log off.”
