REQUIREMENT FOR NEW VALUE ADDED SERVICES (VAS) APPLICATIONS:
1. Duly accomplished Application form for VAS Registration.
2. Description of each of the value added services intended to be offered.
3. List of equipment and materials to be used for the system, including equipment to be installed, if any.
4. Functional block diagram, system configuration and a brief description of the proposed system.
5. Copy of valid facilities/network lease agreement with duly authorized facilities/network providers;
6. Certified True Copy of Securities and Exchange Commission Registration (Articles of
Incorporations and By-Laws) or Department of Trade and Industry (DTI) Registration showing that the entity is at least sixty percent (60%) owned by Filipino citizens.
7. Latest Audited Financial Statement.
8. Schedule of service rates
REQUIREMENTS FOR VOIP APPLICATIONS
Requirements 1, 2, 3, 4, 5, 7, 8 for VAS applications and the following additional requirements:
1. Certified true copy of SEC Registration or DTI Registration showing, among others, that the entity is at least sixty percent (60%) owned by Filipino citizens, and that the paid up capital is at least ten million pesos (P10,000,000.00);
2. Copy of valid interconnection agreements with duly authorized access facilities/network providers and/or leased facilities agreement with duly authorized network provider;
3. List of cities and municipalities where the VOIP service will be offered; and
4. Performance bond of five million pesos (PhP5,000,000.00) from a reputable insurance or surety company duly registered with the SEC and accredited by the Insurance Commission, and preferably from the Government Service Insurance System.
REQUIREMENTS FOR VOIP RESELLERS
Requirements 1, 2, 3, 4, 6, 7,8 for VAS applications and the following additional requirement:
1. Certified true copy of a reseller agreement with duly registered VOIP service provider; and
2. Performance bond of one million pesos (PhP 1,000,000.00) ) from a reputable insurance or surety company duly registered with the SEC and accredited by the Insurance Commission, and preferably from the Government Service Insurance System.
REQUIREMENT FOR RENEWAL OF VAS/VOIP APPLICATIONS :
1. Duly accomplished Application form for VAS Registration.
2. Original Copy of Certificate of VAS Registration.
REQUIREMENT FOR MODIFICATION OF VAS APPLICATIONS :
1. Duly accomplished Application form for VAS Registration.
2. Original Copy of Certificate of VAS Registration.
3. Description of each of the value added services intended to be offered.
4. List of equipment and materials to be used for the system, including equipment to be installed, if any.
5. Functional block diagram, system configuration and a brief description of the proposed system.
NOTE :
Fees for all VAS Registration (except VOIP service Provider and Resellers) and/or Modification:
Filing Fee - P 180.00 per application
Registration Fee (5 YEARS) - P 6,000.00/yr x 5 yrs = P30, 000.00 ( max. of 5
services)
In excess of 5 services - P 1,000.00/per service/per year
Modification Fee - P 180.00
Registration Fee (VOIP Service Provider) - P 50,000.00 per year
Registration Fee (VOIP Service Reseller) - P 5,000.00 per year
Payments of Filing, Registration and/or Modification Fee should be made either in CASH or in
CASHIER’S/MANAGER’S CHECK or DEMAND DRAFT (if out of town) in favor of the NATIONAL
TELECOMMUNICATIONS COMMISSION drawn in any Metro Manila bank/endorsed in any Metro
Manila branch.
