Primary Care Information Project PROJECT COORDINATOR – Community Health Insights The NYC Department of Health and Mental Hygiene (DOHMH), Primary Care Information Project (PCIP) is devoted to improving population health in underserved neighborhoods through health information technology. The Fund for Public Health in New York (FPHNY) is a not-for-profit organization that collaborates with the DOHMH to implement and advance public health initiatives. We are seeking a Project Coordinator – Community Health Insights for a two-year federal grant, funded by the Agency for Healthcare Research and Quality (AHRQ) to evaluate the implementation of the Community Health Dashboard, an innovative software application designed to support cardiovascular prevention programs in faith-based organizations. As a key member of the Administration & Community Engagement team, the ideal candidate must have critical thinking skills and the ability to represent DOHMH and NYU School of Medicine to external partners in the process of planning and implementing the Community Insights Project. Reporting to the Director for Community Projects for PCIP, and in collaboration with NYU School of Medicine, key responsibilities include but are not limited to the following: o o o o o o o Responsible for all aspects of the recruitment cycle including strategy, networking, relationship building, identification of participants and conversion of participants to enrollment into the Community Insights Project Successful utilization of community outreach strategies to recruit faith-based organizations include but are not limited to On-site and off-site meetings with community and faith-based leaders; communication and timely follow-up via email, and telephone; conducting informational sessions and project presentations to faith-based and community leaders as requested; Support faith-based organizations with the recruitment of project participants and the selection of Lay Health Workers (LHWs) to implement and use the Community Health Dashboard Provide support and assist with scheduling, planning and delivery of training sessions Provide ongoing technical assistance regarding the use of the Community Health Dashboard In collaboration with Lay Health Workers (LHWs), customize and implement recruitment strategies appropriate for each faith-based organization through church newsletters, media and service announcements Provide support and assistance to Lay Health Workers (LHW’s) in enrolling participants Coordinate and support PCIP and NYU School of Medicine regarding research components of Community Insights Projects Review memoranda of understanding and all associated documentation with faith-based churches participating in the project Coordinate, plan and assist in the facilitation of focus groups Ensure eligibility of potential participants and conduct informed consent process Facilitate payment and administration of grant incentives to participants, Facilitate baseline and follow-up questionnaires with program participants Assist the Director for Community Projects regarding planning for the sustainability, and expansion of community-based health promotion initiatives for PCIP Participate in health fairs and activities for the community during business hours and occasionally on weekends Provide administrative assistance and other administrative duties as needed. Preferred Education, Experience & Skills o o o o o o Bachelor’s degree from an accredited college or a minimum of 3 years related professional experience Two plus years of experience in project operations, administrative coordination supporting community-based programs; faithbased organizations Familiarity with qualitative and quantitative research methodologies, health information technology applications and the usercentered design process; A Plus Experience facilitating and supporting training sessions and group discussions Excellent project management, administrative time management, attention to detail, organizational, oral, written and communications skills Ability to work collaboratively with internal staff of all levels and external stakeholders o o Intermediate/advanced Microsoft Excel, PowerPoint, Word and Outlook skills Travel within NYC boroughs and the ability to work nights and weekends as required To Apply: FPHNY offers a competitive salary and benefits package.
