PRIMARY CARE INFORMATION PROJECT PRIMARY CARE PHYSICIAN ADVISOR The NYC Department of Health and Mental Hygiene (DOHMH), Primary Care Information Project (PCIP) is devoted to improving population health in underserved neighborhoods through health information technology. The PCIP is the largest project of its kind nationwide, focusing on the efficient use of prevention oriented electronic health records (EHRs) in New York City and works to transform the quality of health care services in these neighborhoods by helping providers use electronic health records and other health technology to improve the quality of care. PCIP also operates the New York City Regional Electronic Adoption Center for Health (NYC REACH) by assisting providers in New York City through practice transformation, quality improvement initiatives and other support services, such as Meaningful Use support, revenue cycle management, and data security. In addition, PCIP has assisted clinicians with: adoption and use of health information technology and exchange integrating electronic health records, communication with patients through a web portal, and the use of EHR derived data for improving the delivery of recommended clinical preventive services practice transformation to meet patient center medical home principles and advanced primary care concepts participation in payer (local, state, or federal) based incentive programs and/or payment reform demonstrations Reporting to the Assistant Commissioner the Primary Care Physician Advisor will be responsible for leading and advising PCIP teams and staff on clinician facing programs. The Primary Care Physician Advisor will: Liaise with content experts and develop road maps for integrating evidence based practices in primary care; lead work groups or advisory panels Lead the piloting and testing of messaging to clinicians with key informants (topics may include: depression screening and treatment, inclusion of community health workers or other patient navigation role, older adult wellness, reproductive health, child health and development) Collaborate with agency leads and partners to connect clinical systems to community resources and establishing bi-directional communication processes Collaborate with teams within PCIP to review and comment on proposed policies/legislation; critical analysis of potential success for new proposed programs/interventions Represent PCIP across the agency and with potential external partners Review and advise on materials to be disseminated to practitioners and where appropriate to patients or stakeholders Provide recommendations for best course of action in engaging practitioners Research, review, and translate clinical evidence based practice recommendations and provide guidance for city wide communication with input from agency leaders and where needed, obtain sign-off/agency approval prior to dissemination Lead dissemination efforts, e.g., agency publications, peer-reviewed manuscripts, presentations at conferences, or internal communication with bureau/agency leaders Preferred Experience Substantial experience as a physician in a primary care setting (e.g., physician owned practice, community health center, hospital affiliated ambulatory clinics) Primary role as principal investigator or program director in clinical demonstration programs or clinical implementation research; experience with health services research or program evaluation Demonstrated experience in planning and developing community clinical partnerships Participated in local, state, or national advisory work groups/steering committees to develop clinical guidelines Familiarity with payment reform, clinical quality measurement, health information technology/exchanges To Apply FPHNY offers a competitive salary and benefits package.
