Go to www.lake.k12.fl.us and click on Departments.
Click on Curriculum & Instruction, Science Home Page and then Secondary Science Fair.
Click on 2014 Forms & Guidelines and then the form you need.
Click on Form 1 Adult Sponsor
 Type your name and your project title
 Check boxes 1-3
 Check the first 4 boxes for #5
 Read for correctness and then print 1 copy
5. Click on Form 1A Student Checklist
 Type your name, grade, e-mail, and phone
 Type the same project title that you used in #4
 Using your provided paper, type the school info. and adult sponsor info.
 For #5, click no
 For #6 refer to your provided paper
 For #7 click home
 For #8, put your name, address, and phone number
 Read through the information and then print the first page only.
6. Click on Form 1B Approval Form
 Type in your name and today's date
 Type in your parent's name and today's date
 Read through the information and then print 1 copy
7. Research Plan - follow the provided directions
 Put your title, name, and school at the top of the paper
 Skip a line between each of the following.
 Type Problem Statement: and then your question being addressed
 Type Hypothesis: and then write your hypothesis
 Type Materials: and then list all materials to be used
 Type Procedures: and then in a list format describe step by step how you will complete
your project
 Make sure you include any safety concerns and how they will be addressed
 Make sure you include how you will analyze the data to find the answer to your
question
 Type Bibliography: and list at least 5 sources plus the one found below
 Use APA format
 Use www.citationmachine.net to check that your format is correct
