Adult Protection Plan

The following is intended to be utilised as an additional tool to the
London Borough of Havering’s Safeguarding Adults Policy, by those
responsible for compiling an investigating officer’s report / holding
meeting.
It is important to bear in mind that this report can be used in subsequent
proceedings and as such is disclosable.
The plan should broadly follow the format below:1.
Outcome of Strategy Meeting / Case Conference if held
2.
Steps taken to ensure future safety of the Vulnerable Adult
(i.e. Action, By Whom, Timescale)

Any reasons it is not possible to increase the safety of the
vulnerable person
4.
Details of support/services etc to be put in place or already existing
(please stipulate which)
5.
Changes to service provision
6.
Strategies to manage anticipated future risk
7.
Support to vulnerable adult if seeking justice/redress
8.
Monitoring and Review Arrangements
(including date for review)
9.
Distribution of this Plan
N.B. Adult Protection Plans must be compatible with any existing or amended Care Plan.
