The Care Coordinator role will be to assist the Manager of Continuing Care Services in the delivery
of the regional Post Acute Care Program (PAC) and the WGHG Interim Care Program.
This is a pivotal role, working in collaboration with the Nurse Unit Managers and District Nursing
Service of our consortia hospitals as well as other health and community services. The program
aims to provide a smooth transition from acute to community care, delivering appropriate and
timely home based services.
West Gippsland Healthcare Group (WGHG) includes an 85 acute bed hospital, a 60 bed Aged Care
facility and District Nursing located in Warragul plus a 50 bed Aged Care facility located in
Trafalgar. The organisation also includes Warragul Linen Service co-located with the hospital in
Warragul and Community Services located in Warragul, Drouin, Erica/Rawson and Trafalgar. The
Post Acute Care office is located in Drouin.
