Care Coordinator – Post Acute Care
Registered Nurse Division 1 Grade 4A or
Allied Health equivalent
Part time – Minimum 16 hours up to 32 hours per week
(negotiable)
The Care Coordinator role assists the Manager of Continuing Care Services in the
delivery of the regional Post Acute Care Program (PAC) and the WGHG Restorative
Care Program (RC) & Transition Care Program (TCP).
This is a pivotal role, working in collaboration with the Nurse Unit Managers and
District Nursing Service of WGHG. These programs aim to achieve a smooth transition
from acute care to community care, delivering appropriate and timely facility and home
based services to assist the recuperation of the client.
A Criminal Records Check will be required for new employees
