Agreements: Please read carefully, initial each section and include with your registration form.
_____ MEDICAL RELEASE WAIVER I certify that I am the parent or legal guardian for the child(ren) I am
registering. I hereby give my permission for any supervisor, coach or other team administrator associated with
the Irving Swimmers, The City of Irving and NorthLake College to seek and give appropriate medical attention for
our child(ren) in the event of accident, injury, illness, etc. I will be responsible for any and all costs associated with
any necessary medical attention and/or treatment.
I hereby waive, release and forever discharge Irving Swimmers and associated supervisor, coach or other team
administrator from all rights and claims for damages, injury, loss to person or property which may be sustained or
occur during participation in Irving Swimmers activities, whether or not damages or loss is due to negligence. I
hereby acknowledge that my children is (are) physically fit and capable of participation in all Swim Team activities.
_____ LIABILITY WAIVER By registering my child(ren) with the Irving Swimmers, I agree to participate (or allow
my child(ren) and family members to participate) in the Irving Swimmers, and hereby release Irving Swimmers, the
City of Irving and NorthLake College, their directors, officers, agents, coaches, and employees from liability for any
injury that might occur to myself (or to my child(ren) and family members) while participating in the Irving
Swimmers program, including travel to and from training sessions, swim meets or other scheduled team activities.
I agree to indemnify and hold harmless the above mentioned organizations and/or individuals, their agents and/or
employees, against any and all liability for personal injury, including injuries resulting in death to me, my child(ren)
and/or other family members, or damage to my property, the property to my child(ren) and/or other family members,
or both, while I (or my child(ren) or family members) participate in the Irving Swimmers program.
_____ DUES, FEES AND OTHER CONSIDERATIONS Monthly Dues: In consideration of the participation of
the member in Irving Swimmers' swim program, the account holder agrees to pay the dues for the member's practice
level that is set forth on the Fees and Member Obligations. Monthly dues are payable September through July (with
no payment in August except outstanding balances). Dues may be prepaid at any time. Invoices shall be sent by IS
via email on the 1st of each billing month and payment in full by mail or by drop box at NorthLake Aquatic Center is
due by the 10th of the same month. Late payment fee is automatically charged by the billing system. In any given
month, if you decide to pay just a partial amount of the total outstanding balance, the billing system will still charge
the full late fee to the account holder. Monthly dues are not prorated except when a member is transferred to a
different team within the Irving Swimmers association by the coaching staff.
Annual Membership: An annual membership fee for each swimmer on the family account is collected in September
of each year. Siblings pay the annual membership fee on a discounted scale as set forth in the Fees and Member
Obligations. Members who join within the year may pay an annual fee based on a sliding scale as described in the
Fees and Member Obligations.
Fundraising obligation: IS requires per member payment for annual fundraising. If the full amount is not raised in
the official fundraising activities offered by the team, the account holder will be invoiced for the difference (per
member) on the June billing invoice or upon leaving the team. Deactivation from the team does not release the
member from these obligations.
Volunteer Service Obligation: Each account holder is required to provide at least ten (10) hours of service to help
the team thrive. These hours may be provided by any member of the account or by extended family. A per hour fee
will be assessed on the June billing invoice for any volunteer hours not fulfilled. There are numerous opportunities
offered throughout the year for volunteering as well as many ongoing positions with the team. If a member joins the
team in the months of April through July, the required hours are reduced to five (5). Unfulfilled hours are billed to the
member's account on May 31. Members who join April through July have until July 31 to fulfill their reduced hours.
USA Swimming Membership: Every member of the Irving Swimmers swim team (including pre-competitive) must
be a member of USA Swimming. This is a separate membership and fee apart from Irving Swimmers.
Leaves of Absence: Non-medical leaves of absence for 65 days or less require continued payment of monthly
dues. If the leave is 66 days or more, the member terminates membership and rejoins paying the annual member fee.
ALL non-medical leaves require a 30-day notice to the Treasurer in written form (email is acceptable). Medical
leaves require a note from a medical doctor to waive monthly dues up to 65 days. If the medical leave is 66 days or
more, the member must pay the annual membership fee to rejoin. Medical leave does not require a 30 day notice.
Summer Leaves of Absence: Summer leave of absence and suspension of monthly dues will be allowed under the
following conditions: 1) the requested leave is in the time period between June 1 and July 31 only; 2) the duration is
no less than 30 days and no more than 60 days; 3) all commitments and obligations are in good standing, including

volunteer hours and fundraising commitment; 4) dues are current before the actual start date of the leave; and 5) the
member agrees to pay a $25 per month account maintenance fee.
Voluntary Termination: If a member voluntarily leaves the team for any reason at any time of the year, a 30 day
notice must be given to the Treasurer in written form (email is acceptable). If no notice is given, one month's
membership dues will be assessed. Account holder for that member is responsible for any outstanding balance upon
termination. Unpaid amounts will be considered delinquent if not paid by the last day of the month in which it is due,
regardless of continued membership with Irving Swimmers. IS reserves the right to notify any and all other USA swim
teams of an account holder's delinquent account.
Delinquent Accounts: Each month, an invoice is emailed to the account holder for monthly dues and any other
assessments (swim meet fees, spirit wear, etc). This invoice is due by the 10th of the month. A late fee is assessed
after the 10th. If an account remains unpaid by the last day of the month in which it is due, a notice of delinquency
will be mailed (and emailed) to the account holder. If the account holder fails to pay in full within 14 days from the
date of written notice of delinquency, all members under that account shall be suspended from further participation in
all IS activities including, but not limited to, practices, events and meets. If the account holder becomes delinquent in
payment because of financial hardship, he/she may apply to the Treasurer for a waiver of late fees and suspension
and arrange a plan for payment of the remaining outstanding balance.
High School Swimmers: Any member of Irving Swimmers who is concurrently active on a high school swim team
may apply for reduced monthly fees with Irving Swimmers, usually 1/2 of the monthly dues rate. To receive the
reduced rate, the member must submit a Request for Reduced Fees form signed by the high school coach and is
then limited to two (2) practices per week plus the Saturday practice. This rate is applicable only during the high
school swim season typically September through March. Account holders are still responsible for the full cost of any
other fees including, but not limited to, swim meet fees, annual fee, fundraising obligation, volunteer obligation and
USA Swimming membership.
Other Monetary Responsibilities: Account holders are responsible for any charges or purchases incidental to the
following: swim meet fees, spirit wear, swim caps, swim suits (competition or otherwise), goggles or any other
equipment, team travel expenses. Some items are required for competition and training but are not supplied nor paid
for by Irving Swimmers.
_____ PUBLICITY PERMISSION Account holder grants Irving Swimmers the right to use, reproduce and/or
distribute photographs, films, videotapes electronic images and sound recordings of account holder and any member
of that account, without compensation or approval rights for use in material created for the purposes of promoting the
activities and knowledge of Irving Swimmers.
_____ CODE OF CONDUCT AGREEMENT All youth members of Irving Swimmers, Inc. agree to abide by the
following standards during practice, meets/competitions or during any other team function or event. In addition, any
standards set forth by USA Swimming will take precedence and must be followed. As the member's parent, you are
agreeing that you have discussed these standards with your swimmer(s).
1. The possession or use of alcohol, tobacco, or any non-prescribed drug is prohibited.
2. The possession or use of any firearm, hazardous chemical, or pyrotechnic (like fireworks) is prohibited.
3. All athletes are expected to follow the directions of the coaching staff.
4. Improper or destructive behavior will not be tolerated. The swimmer or swimmer’s family will be responsible for any
damage expense incurred to IS equipment or that of NorthLake College.
5. Abusive language or conduct directed at the coaching staff, parent, chaperones, or team members will not
be tolerated.
6. There shall be no inappropriate physical contact between athletes or between athletes and coaches.
Additional requirements for team sponsored Travel Trips include:
1. Curfews established by the coaching staff will be adhered to each day.
2. Swimmers are required to attend all team meetings, team functions, or meet sessions unless excused by coaching
staff.
3. Swimmers under the age of 18 must be accompanied by another swimmer or chaperone outside of hotel rooms at
all times.
Failure to comply with the Code of Conduct may result in, but not necessarily be limited to, the swimmer
being removed from practice or team function/meet, and will be sent home with the parent or at the parent’s expense.
Upon notification of any violation of the Code of Conduct, a review committee, consisting of the coaching staff and the
Irving Swimmers, Inc. Board of Directors, shall promptly investigate the circumstances of the violation, shall notify the
individual(s) charged at the time for a hearing, and shall conduct an informal hearing. The review committee shall
then promptly determine what disciplinary action shall be taken.
