HESI - MTSU Nursing Admission Assessment
The HESI Admission Assessment Exam must be taken at MTSU’s testing services or a Prometric Testing Center.

Prior to the testing appointment

Go to https://evolve.elsevier.com to create an Evolve account. You will be issued a username and password for the
test.
An account must be created to view and print scores to be included in the application packet to the MTSU School of
Nursing.
To Register

Call Testing Services at 615-898-2863 to make an appointment after registration is complete
•
You may register in person at the Keathley University Center, Room 107
Items required at testing appointment

Evolve account username and password - very important
Unexpired driver's license OR passport
•
A four-function, scientific, or graphing calculator (except the TI-89) may be used on the
HESI.
Additional Information
•
You must sign up for ALL SUBJECT PARTS of the exam. See box titled “HESI
Subjects”.
•
You are required to wait thirty (30) days before retaking the test.
•
Exam is offered on individual computers.
•
Appointments are most weekdays at 8:00 a.m.
•
Cost is $60.00 plus a $3.50 online convenience fee.
•
There is a four (4) hour time limit.

The information provided must be the same as it appears on your government issued photo ID. You will not be
admitted into the testing lab if there is any discrepancy between what it is entered into the system (by Elsevier) and
what is presented on your ID. Please only submit your Eligibility ID request once.

Please check with the selected Prometric center for identification requirements. Middle Tennessee State University
Department ID: 198769.

You will receive a Confirmation Notice indicating that you may schedule your exam. Please carefully review the
information contained in your Confirmation Notice. If any of the information is incorrect, or if the information has
changed, please contact Elsevier Review and Testing at exameligibility@elsevier.com. Failure to do so could result
in forfeiture of your testing appointment and associated fees.
